Objective To detect the expressions of three histone deacetylases (HDAC1/HDAC5/HDAC6) in tissues of benign prostatic hyperplasia(BPH) patients, and to research the function of HDACs in the pathogenesy and progress of BPH.Methods (1) The expression of HDACs was assayed by immunohistochemical technique. Tissues are from 5 normal prostates(NP) and 60 patients with pathologic diagnosis of BPH after the surgery of transurethral resection of prostate (TURP). Expression levels were scored as strongly, moderately, weakly positive and negative according to the different staining levels.(2) All of the BPH patients’prostate volumes were measured by MRI before TURP and were divided into 3 groups: A(25~50ml,15cases),B(50~75ml,25cases)and C(>75ml,20cases). Statistical method was performed to analyze the dependability between total prostate volumes and levels of HDAC1/HDAC6. (HDAC5 was excepted because there is no significant difference of expression between BPH and NP groups)(3) The BPH cases with acute urine retention(AUR) were reviewed,and the relationship between AUR’s incidence and levels of HDAC1/HDAC6 was analyzed by statistical method. (HDAC5 was excepted because there is no significant difference of expression between BPH and NP groups)Results (1) The positive expression of HDAC1 was 68.3%(41/60) in BPH tissues, with 3 strongly positive,21 moderately positive and 17 weakly positive. Four-fifths of the NP tissues were detected as negative, with only 1 weekly positive. HDAC1 levels in the tissues of BPH were higher than those in NP tissues significantly (P<0.05).(2)The positive expression of HDAC5 was 23,3%(14/60) in BPH tissues, Four-fourths of the NP tissues was detected as negative, with only 1 weekly positive. Levels of HDAC5 were not statistical significant between the two groups (P> 0.05).(3) The positive expression of HDAC6 was76.7%(46/60) in BPH tissues, with 4 strongly positive,23 moderately positive and 19 weakly positive. Three-fifths of the NP tissues were detected as negative, with only 2 weekly positive. HDAC6 levels in the tissues of BPH were higher than those in NP tissues significantly (P<0.05).(4) In volume groups A,B and C, the HDAC1 weakly positive expression rates were 26.7%,28% and 30% respectively; moderately positive expression rates were13.3%,28% and 60% respectively; strongly positive expression rates were 6.7%,8% and 5% respectively. The rates of HDAC1’s moderate expression significantly increased with the enlargement of prostate (P<0.05).(5) In volumes groups A,B and C, the HDAC6 weakly positive expression rates were 33.3%,32% and 30% respectively; moderately positive expression rates were 40%,36% and 40% respectively; strongly positive expression rates were 6.7%,8% and 5%. Levels of HDAC6 were not statistical significant among the three groups (P> 0.05).(6) 22 cases of BPH once suffered AUR and urethral catheterizations. In HDAC1 group, the rates of strongly, moderately, weakly positive and negative expressions were 66.7%(2/3),47.6% (10/21),41.2%(7/17),15.8%(3/19) respectively. Compared with HDAC1 negative cases, HDAC1 positive cases had a higher incidence of AUR compared with negative group (P<0.05), and the AUR incidence significantly elevated with the increasing of HDAC1 levels detected in the BPH tissues (P<0.05). In HDAC6 group, the rates of strongly, moderately, weakly positive and negative expressions were 25% (1/4),39.1%(9/23),36.8%(7/19) and 35.7%(5/14) respectively. Levels of AUR incidence were not statistical significant among the HDAC6 positive and HDAC6 negative groups (P> 0.05).Conclusion (1) HDAC1/HDAC6 expressions in the tissues of BPH were higher than those in NP tissues significantly.(2) The levels of HDAC1’s moderately expression significantly increased enlargement of prostate volume. Meanwhile, the AUR incidence significantly elevated with the increasing of HDAC1 levels.(3)HDAC1 may play a key role in the pathogenesis and progress of BPH, it may be a novel therapeutic target in BPH.
