Objectives:In this research, we utilize electrically heated stone needle therapy, to intervene the treatment of patients,who suffering from sepsis with gastrointestinal dysfunction. By means of monitoring their gastrointestinal index, inflammation index, degree of critical condition and prognosis index, and safety index, to observe the clinical effect and safety of electrically heated stone needle therapy on sepsis with gastrointestinal dysfunction. And reveal its mechanism of action. Therefore, provide a convenient, safety and efficient clinic method for sepsis with gastrointestinal dysfunction.Methods:In this research, totally 90 example cases of ICU department in Guangdong Hospital of Traditional Chinese Medicine, dated from Aug.2014 to Dec.2015, was selected. All the cases meet the diagnostic critical diseases with sepsis and gastrointestinal dysfunction.90 patients were divided into the observation group (n=45) and control group (n=45) randomly. The patients in the observation group were offered electrically heated stone needle therapy based on routing treatment and nursing, while patients in the control group offered routing treatment and nursing with abdominal massage. Patients of the two groups were intervened for 5 consecutive days before eating in half an hour, twice a day, respectively in the morning and afternoon. Their gastrointestinal index (gastrointestinal dysfunction score, the tolerance of enteral nutrition, bowel sound, stool frequency, and abdominal circumference), inflammation index (the leukocyte count, C-reactive protein level, and procalitonin level), degree of critical condition and prognosis index (APACHEIIscore, Incidence of MODS, the 28 days morality and ICU stay time), and safety index were monitored after intervention, to evaluate the clinical effect and safety of electrically heated stone needle therapy on sepsis with gastrointestinal dysfunction.Results1.There were 90 patients enrolled in the study, each group for 45 ones. During the research, there were two patients dropped out from the observation group, one of them was because of death, and one refused to cooperate. In the control group, there were three patients dropped out, one of them was transported to other hospitals for therapy, one was because of death, and another one was because of disease progression. While others of the two groups all finished the study well. Therefore, in fact, there were 85 patients enrolled in the study, with 43 ones for the observation group and 42 for the control group.2. Before the intervention of this research, the two groups were well balanced in the common condition, including sex, age, degree of critical condition on sepsis, underlying diseases and infection sites, and there were no significant differences between the two groups (P>0.05). In the aspect of gastrointestinal index, after intervention for 5 days, the gastrointestinal dysfunction scores of patients in the observation group were better. Whether the ITT analysis or the PPS analysis, there were significant differences between the two groups (P<0.05); their bowel sound and stool frequency were also improved after the intervention for 5 days, and compared with the control group (P<0.05); the tolerance of enteral nutrition was also improved.while compared with the control group(P>0.05); the abdominal circumference was reduced, but compared with the control group(P>0.05), too. In the aspect of inflammation index, the leukocyte count, C-reactive protein level and procalitonin level of the patients in observation group were decreased after the intervention for 5 days, and compared with the control group(P<0.05). In the aspect of the degree of critical condition and prognosis index, the APACHE Ⅱ scores of patients in the observation group were reduced after the intervention for 5 days, and compared with the control group(P<0.05); The incidence of MODS and the 28 days morality were lower than the control group, and ICU stay time was shorter than the control group, but there were no significant differences between the two groups (P>0.05). In the aspect of the safety index, there was no obvious discomfort or other adverse reactions between the two groups during the intervention.Results1. Electrically heated stone needle therapy can enhance gastrointestinal peristalsis, and reduce defecation and anal exhaust time. What’s more, it can also improve the tolerance of enteral nutrition and is good for the rehabilitation of sepsis with gastrointestinal dysfunction.2. The circulation in local pathological tissues can be improved by electrically heated stone needle therapy, and so was the inflammation index and SIRS.3. Electrically heated stone needle therapy can improve the degree of critical condition and prognosis, reduce the morality and shorten the ICU stay time.4. Electrically heated stone needle therapy is convenient, effective and safe, which can solve the problem of oral or nasogastric administration of sepsis with gastrointestinal dysfunction, and it should be widely used.
