Study On Dalian Municipal Central Hospital, Home Care Mode Of"Six In One"

China has entered an aging society, while most areas will rapidly emerging family miniaturization trend under the family planning policy. Therefore, the basic desire of children to take care of the elderly can not be achieved. The social pension in the future will be the inevitable choice of many elderly. The particularity of the physical and mental health of the elderly depends on the combination of medical and pension to meet basic needs, as well as the issues of spiritual solace and social reintegration also need more social attention. Home care combined with existing community resources means that strengthening the integration of institutional pension and family pension. The new mode system has been given to improve innovation pension system, improve the quality of life of elderly life and mission. It adapts to China’s national conditions and aging society. Currently, public hospital is undertaking social pension. It not only conform the policy guidance, but also play an important role in improving the pension system, and promoting quality of life of elderly. The policy guidance includes "to promote the development of social forces to become the protagonist of pension services" and "to promote the convergence of the medical support, medical institutions to explore new modes of cooperation with pension institutions". Dalian Central Hospital hosted home care service centers to provide medical, nursing, medical psychology, medical nutrition, social adaptation and living a "six-one" integrated services. And the services combined lots of forms, i.e., short, medium and long-term stay, hosting and day care, and home. It provides45kinds of medical and31kinds of nursing services, as a non-profit public service organization. After nearly a year of operations, the hospital summed pension experience, brought together the social repercussions, adjust concept ideas, and improve operational mode. And then, it supports comprehensive services for elderly to maintain social contacts and enjoy a happy old age. In this study, we will explore how public hospitals to undertake community home-based care model. And then, it can be replicated in order to reach a good operating result. Besides, it will cost3-5years, and will works well in the district of Dalian everywhere, solve more social pension problem.
