With the acceleration of an aging society, the elderly pension and medical problem increasingly prominent in our country. The original old-age service system is facing tough challenges. Therefore, Chinese government proposed a new pension form of "combined medical and nursing based on the demand of the elderly. The idea is to integrate the pension and health care resources and incorporate the elderly pension service demand and medical service demand into a unified standard system, enhancing the level of integration between modern social pension and medical care. From community-based perspective, this paper analyzed the elderly demand, policy implementation, resource links, and the specific service of the "combined medical and nursing ", achieving a preliminary investigation of combined medical and nursing "service.This study used a combination of qualitative and quantitative research methods and applied the literature data collection method. Through the secondary analysis of the 500 elderly samples collected from eleven blocks in Xuanwu District, we examined the current "combined medical and nursing" situation of the Xuanwu District and analyzed the level of discrepancy between supplying and demanding. On this basis, our research listed a specific "combined medical and nursing" project to microscopically describe the process of "combined medical and nursing’" form in Xuanwu District. After reviewing both macro and micro aspects of community care, we summarized the current characteristics of combined medical and nursing" service in Xuanwu District of Nanjing, enabling the better analysis of current dilemma and the possibility of a further development in the future.The results showed that the main factors of "Combined Medical and Nursing" service in the community care for the elderly are educational level, income level, physical condition, and etc. There is still a wide space for the community to enhance the "Combined Medical and Nursing" services. In this paper, Xuanwu District’s" Combined Medical and Nursing services are summarized as three types service: service based on community care centers, service based on community health centers and service based on the introduced commercial oriented operation.This study conducted a SWOT analysis to deepen the medical support integration of the community development, with advantages in that the "Combined Medical and Nursing" service has a high starting point and distinguished feature and the urban policy is in line with the public opinion. The disadvantages of the "" service is the single form, lack of financial and human resources, and weakness of coordination mechanisms. This new form of service is facing the opportunities of the incoming "medical" policy and "silver economy". Possible risks are the institutional difficulties and the associated high cost. Based on this analysis, this study has brought the suggestions as follows:First, to take advantages of the local experience and establish the " Combined Medical and Nursing " service model; Second, to break the institutional barrier and develop the "Combined Medical and Nursing" access system; Third, to seize the policy opportunities and drive the "Combined Medical and Nursing" pattern into a deeper and broader operation and development.This article is structurally divided into six parts. The first chapter is the introduction, and summarized " Combined Medical and Nursing " service problems and significances; the second chapter is the theoretical basis and literature review; The third chapter is the analysis of research methods and research framework; The fourth chapter is the interpretation of research data; in the fifth chapter, the author has carried out the SWOT analysis to Xuanwu District’s" Combined Medical and Nursing " services; The sixth chapter provides suggestions and countermeasures to promote and deepen the integration of medical and nursing in the community.
