Study On The Medical-nursing Combined Care In Community In Jianghan District In Wuhan

This study starts from the community level of medical-nursing combined care service,then take the representations of the service organization of different ways in medical-nursing combination and the key suppliers of medical-nursing combined care service in Jianghan district in Wuhan as the research objects,and understand the development present situation,existing problems and reasons of medical-nursing combined care service in communities of this region,as well as put forward suggestions on perfecting the medical nursing combined care service,so as to enrich the research achievements in the field of medical-nursing combined care,as well as provide a scientific basis for the government to promote the medical-nursing combined care service for the elderly comprehensively in next step.This paper is divided into four parts:Introduction is the first part of this paper.It briefly explains the background and significance of this study,analyzes the research status at home and abroad,as well as illustrates the purpose of this study,research methods,innovation points and the technical route.The second part is about the related concepts,theory and practice of medical nursing combined care.It defines several important concepts such as pension service,medical-nursing combined care,and modes of medical-nursing combination at first.Then,it briefly expounds the theoretical basis of this study,including the theory of demand level,the theory of multiple benefits,as well as the theory of public goods.In the practice part of medical-nursing combined care,the ways of medical-nursing combined care is divided into the integrated mode group and cooperative mode group according to whether the cooperation between medical institutions and pension institutions involves in property adjustment.Furthermore,it introduces the good experience in developing medical-nursing combined care service in communities in United States,Britain and Japan respectively.The third part analyzes the characteristics of medical-nursing combined care service in communities in Jianghan district in Wuhan,and summarizes the existing problems and their reasons.The characteristics can be summarized as:the public-operated nursing home drive the development of medical-nursing combined care as the leading;there are integrated mode and cooperative mode of medical-nursing combined care existing in nursing homes of communities;the community health service institutions play an important role in medical-nursing combined care;as well as individual signing service of medical-nursing combined care are provided to the elderly living in community and home.The current existing problems include:the low end of the level of medical-nursing combined care service for private-operated nursing homes,the shortage of human resource of medical-nursing combined care service,the economic burden of medical-nursing combined care service for the elderly,as well as the lag and conflict of supporting policies of medical-nursing combined care from government departments.The principal causes of these problems are:health service conditions of private-operated institutions are congenitally deficient;the industry of pension service is limited appeal to professional talents;the existing medical insurance policies are with restriction and it is lack of long-term care insurance system,as well as collaborative mechanism of medical-nursing combined care service has not yet been straighten.The fourth part puts forward the perfect measures and suggestions on perfecting medical-nursing combined care service in communities in Jianghan district in Wuhan..Including:to improve the health service level of nursing homes and family in community,to construct service personnel of medical-nursing combined care service,to build a multi-level social security system for the elderly care,and to promote the integration of old-age service resources between health and civil affairs.
