Objective:To investigate the susceptibility gene ORMDL3 single nucleotide polymorphisms in infantile wheezing, and analyze Tidal breath pulmonary function? FENO clinical indicators,providing the evidences for the early diagnosis of infantile asthma.Method:68 infants aged 6~36 months were selected from Department of Pediatrics,Provincial Hospital Affiliated to Shandong University,according to the API were divided API positive group(33 infants) and API negative group( 35 infants).API negative criteria: children under 3 years old and, at the same time meet the following five, namely:(1) no family history of asthma and allergic diseases atopic dermatitis(2) Non-atopic dermatitis(3) no inhaled allergen sensitization basis(4) no food allergen sensitization(5) basis peripheral blood eosinophils <4%.Two groups of children drawn fasting blood 3ml, Taq Man probe method by genotype analysis of ORMDL3 SNP site rs4794820, rs7216389;Exhaled nitric oxide(FENO) measuring the concentration of selected yet Wonakulun nitric oxide analyzer, using off-line methods to collect exhaled gas is measured; tidal pulmonary function detected by Jaeger spirometer, the main parameters: the time to peak ratio(TPTEF/TE), peak volume ratio(VPEF/VE).Comparison of rs4794820 and rs7216389 polymorphism loci distribution difference between API positive and negative groups, and clinical and laboratory examinations were analyzed. Result:1.68 children were enrolled wheezing, 33 cases in API positive,( 28 boys, five girls), age(22.83 ± 9.31) months; API negative group 35 cases( 26 boys, nine girls), the average age( 22.37 ± 11.49) months.Two groups of age, sex showed no statistically significant(> 0.05);2.In API positive and API negative group, there are differences different genotype rs4794820 and rs7216389 locus(p < 0.05); Multivariate Logistic regression analysis showed that the distribution of rs4794820 polymorphism loci, compared with GG genotype, AG genotype could significantly increase the incidence of asthma(2? = 17.217, p = 0.000, OR = 19.091); the rs7216389 polymorphism distribution, compared with the TT genotype, TC genotype could significantly increase the incidence of asthma(2? = 6.574, p = 0.01, OR = 5.236);3.API positive group TPTEF / TE, VPEF / VE were lower than the API negative group( t = 4.21, p = 0.0001; t = 6.26, p = 0.000); the API positive group, while expressing AG / TC genotype children TPTEF / TE is lower than at the same time expressing GG / TT genotype patients( t = 2.31, p = 0.038), the difference was statistically significant,but VPEF / VE was no significant difference(t= 2.00, p = 0.08);4.API-positive children FENO concentrations higher than negative group, the difference was statistically significant( t = 5.36, p= 0.0000); in the API positive group, FENO concentration expressing AG / TC genotype and the simultaneous expression of GG / TT genotypes among children with no difference(t= 0.557, p = 0.587);5.TPTEF / TE and FENO( r =- 0.101, p = 0.462), VPEF / VE and FENO( r =- 0.023, p = 0.867) no correlation.6.EOS%? total Ig E in API positive group were higher in API negative group( t =3.59, p = 0.001; t =2.84, p =0.01),and EOS% with FENO was positive correlation( r=0.788, p=0.000);7.Blood serum allergen test found that indust mite and dog hair were accounted for the largest ratio in inhalation group. Food group as followed Sea fish? Freshwater fish, Egg protein? soy;8.Respiratory viruses RNA test,HRV was highest,followed by RSV?Abv9.X-RAY? CT were no significant in two groups. Conclusion:1.ORMDL3 single nucleotide rs4794820 AG genotype, rs7216389 TC genotypes are susceptible genotype Infant; rs4794820 GG genotype, rs7216389 TT genotype is genotype protection.2.There are different degrees of damage lung function in children with wheezing, API positive group than negative group obviously;in the API positive group, while the expression of children AG / TC susceptible genotype than the simultaneous expression of children with TT / GG genotype protection the heavy lung damage.3.API positive airway inflammation in children with levels above API negative patients;4.Tidal lung function and FENO no correlation;5.Inhalation of dust mites in combination group, the most common dog epithelium; ingestion group with a combination of fish, freshwater fish, egg protein, soy common.6.Wheezing infants in the incidence of respiratory viral infections to rhinovirus, syncytial virus common.
