Catgut Implantation At Acupoints For Allergic Rihnitis On Involved Meridian:a Randomized, Controlled Trial And A FMRI Study

Objective:Asessmentthe clinical effectiveness of Catgut implantation at acupoints forAllergic Rihnitis on involved Meridian. On this basis,we preliminary explore the central response characteristics for the treatment of AR Catgut implantation at acupoints,and try to provide scientific experimental basis for the clinical application of treatment of AR.Methods:1.We using Single-blinding, randomized(computer random number table method)and controlled clinical research method including in 30 patients that meet our inclusion criteria,and those patients were randomly divided into experimental group (group A)/(group of catgut implantation at acupoints) and control group/group B (group of fake catgut implantation at acupoints). According to select point on Involved Meridian principle we do catgut implantation at the same acupoints in group A and group B, but the only difference is no catgut is implantationed in acupoints in Group B just only a needle stimulation. In total,1 time catgut implantation at specificacupoints and 2 times follow-up (the second week and the sixth week) to be done. We do index observation before and after treatment for 2 weeks and 6 weeks, and using RQLQ score, VAS score, nasal symptoms, and nasal endoscopy and serum index as effectiveness evaluation. The index observation results will be as evaluation to assess the effectiveness and safety of catgut implantation treatment.2. We randomly drawing 5 samples that meet our inclusion criteria in group A in the clinical research, and using brain functional magnetic resonance imaging (fMRI) technique scan those 5 samples and apply fMRI image as the research outcomes. By comparing the image before and after and screening key (target) brain regions associated with illness,we summarize and discuss maincentermechanismunder the treatment of catgut implantation at specificacupoints?Results:1. The results of catgut implantation at acupoints for Allergic Rihnitis on Involved Meridian principal(1) The demographic characteristics of 2 groups of participants, vital signs and baseline data is comparable.(2) The nasal VAS score item after 6 weeks, statistically significant difference between the two groups.(3)The RQLQ score at the second week and the sixth week after treatment have statistical significance differences between the two groups.(4) The inferior turbinate swelling degree score at the second week and the sixth week after treatment have statistical significance differences between the two groups.(5) The before serum index of IFN of group A comparing to the second week treatment was statistically significant.(6)This trial showed no serious adverse reactions.2. The study outcome of Resting state of fMRIBefore and after catgut at acpoints,the contrast significantly enhance regional areas of the brain are:both sides of the calcarine (BA17), anterior cingulate cortex (ACC) (BA32),middle cingulate cortex (MCC) (BA23), posterior cingulate cortex (PCC) (BA30),lentiform nucleus, cuneus gyrus (BA23),the insula (BA13), superior temporal gyrus (STG) (BA41),middle temporal gyrus (MTG) (BA21), interior temporal gyrus (ITG) (BA20), inferior frontal gyrus (IFG) (BA47), middle frontal gyrus(MFG) (BA10), superior frontal gyrus (SFG) (BA8), anterior central gyrus (BA4), central precentral gyrus (BA3), premotor cortex (BA6), edge (BA40), angular gyrus (BA39),Cerebellar tonsillar,left caudate nucleus, left fusiform gyrus (BA37).Conclusion:(1) Group A AR curative effect is better than that of group B, Catgut implantation at acupoints for Allergic Rihnitis on Involved Meridian is safe and effective treatment.(2) Catgut implantation at acupoints for Allergic Rihnitis on Involved Meridian have therapeutic Central effect mechanism.(3) Catgut implantation at acupoints for Allergic Rihnitis on Involved Meridian has the following features: ?Generalized transfer the central brain regions. ?Has the characteristics of the targeting property. ? Has the characteristics of the pioneers before the clinic.(4) The clarity of the mode of central effect path or effect in remains to be further research.(5) Hypothesis "acupuncture point-the neural signal conditioning-central nerve signal conditioning organs-" has a certain credibility.
