Part 1.The Risk of Gestational Diabetes Mellitus(GDM)in the Different Age of PregnancyObjective The increased gestational age are closely related with GDM,but it was not to be payed enough attention to how it affects GDM since the age of pregnancy increased recently.We study the effect of the age of pregnancy on the prevalence of GDM.Methods The age and glucose levels during OGTT of 21371 pregnant women who were given a single live birth in Obstetrics and Gynecology of the 306 Hospital of PLA from 2007 to 2015 and did one-step 75 g OGTT during 24 to 32 gestational weeks were reviewed.The relationship between pregnant age and glucose levels during OGTT were analysis,the effect of pregnant age on the prevalence of GDM were assessed in accordance with ADA diagnostic criteria 2011.Results The age of GDM is higher than the age of NGDM pregnant significantly(33.28 ± 5.36 years vs.29.67 ± 3.58 years,P<0.05).Pregnant age were positively correlated with the fasting,60 min,120 min glucose levels during OGTT(r=0.037,0.083,0.095,P<0.000).GDM prevalence increases with pregnant age,~ 20 years old,~ 25 years old,~ 30 years old,~ 35 years old,~ 40 years old,~ 45 years old,GDM prevalence rates were 0%?5.1%?7.5%?13.5%?30.4% and 68.5%.2007-2015 years,the average age of NGDM and GDM increased year by year,the average age of GDM pregnant women increased from(32.57 + 5.52)years old to(34.57 + 4.95)years old,the average age of 9 years increased by about 2 years old.The peak age of the pregnant women and the NGDM were 29 years old,while the GDM peak age was 31 years old.Conclusion Increased pregnant age may pay important role to the high prevalence of GDM in the recent 9 years.It is necessary to pay attention to the high prevalence of GDM in the pregnancy younger than 30 years old.Part 2.Changes and clinical significance of serum glucagon like peptide-1 during 24 to 28 gestational weeks in gestational diabetesObjective To investigate the changes and clinical significance of serum glucagon like peptide-1 in patients with gestational diabetes mellitus..Methods40 cases of gestational diabetes mellitus(age 32.69 ±5.46,gestational week24.18 ± 1.83)and 42 cases of normal pregnant women(age 30.45 ± 2.72,gestational week 24.38 ±0.88)have been enrolled.Determination of serum GLP-1 using 75 g OGTT during 0,30,60,120 minutes in pregnant 24-28 weeks,compare the changes and analyze the relationship between the GDM group and normal group.Results No significant changes of serum GLP-1 in GDM during 75 g oral gulcose test(OGTT)were found.However the peak of GLP-1 level during OGTT was at 120 min in GDM,which was 30 min in NGT.Insulin(INS)/ GPL-1 ratio at 30 min decreased significantly in GDM(38.37±32.77 vs 69.55±69.43,P<0.05).Fasting GLP-1 and AUCGLP-1 are positively correlated to AUCINS(r=0.24,r=0.27,p<0.05).Conclution Our results sugessted insufficient serum levels of GLP-1 in GDM in the early stage during OGTT.
