[Objective]To explore the relationship between hypertension and lead load using a case-control study in Lead-Zinc mining areas of Lang ping of Yunnan province,China.[Methods]Based on the previous studies and the actual situation of lead pollution in Lead-Zinc mining areas of Lang ping of Yunnan province,China.1:1 matched case-control study is used in this study.Matched for cases and controls the same sex and age difference less than 0.5 years.The case group select the patients who have been in Jin ding and Tong dian townships of Lan ping County for three years and above.Meanwhile,these cases are hypertension diseases or not taking antihypertensive drugs under the condition of blood pressure 140 mmHg and blood pressure 90 mmHg as the case group,but excluding secondary hypertension.The control group is far from the mining area,industrial pollution and no water contact.The participants are selected from Jing hua and Da yang village of He xi twonship as the control group.The participants whose life habits,,socioeconomic status is basically as same as case group in the mining area in Lan ping county.These participarts without hypertension ever and the blood pressure less than 140 mmHg or 90 mmHg.The questionnaire that is design by the research team is used to collect personal information including two group population health status,blood pressure,height and weight and so on.The blood and urine samples of two groups participants are tested using the standardized methodin the laboratory of School of Public Health.Data were entered using Epitata3.1softeware and statistical alanalysis using SPSS 17.0.[Results]1.To compare the relationship between the level of lead load and the subjective symptoms of the case group and control group,we reveal that lead load is associated with headache,joint pain,dizziness,abdominal pain,numbness of limbs.2.Comparing incidence of headache,joint pain,dizziness,abdominal pain,numbness of limbs,the difference between the case group and control group is statistically significant(P<0.05).Incidence rates of headache,joint pain,dizziness,abdominal pain,numbness of limbs is higher than that in control group.3.In cases blood lead content range is 80.1 ~ 401.25μg/L,the median is120.35gμ/L,the control group blood lead levels ranging from 30.13 to 80.46μg/L,the median blood lead content of 45.16μg/L.Two group comparison test results of Z=-12.271,P=0.000,the difference is statistically significant(P<0.05).Cases of urinary lead content in the range of 4.350 ~ 79.16μg/L,the median is 65.17μg/L.The control group of urine lead content in the range of 3.425 ~ 55.54μg/L,median lead content 47.15μg/L.Two group comparison test results of Z=-8.222,P=0.000,the difference is statistically significant(P<0.05).Blood lead group cases,urinary lead levels higher than the control group.4.By univariate analysis,27 of the project,found the case group and the control group family history of hypertension,family history of cardiovascular and cerebrovascular diseases,varieties of edible oil,wine,tea,attitude and high blood lead content with statistical differences in blood lead level.Hypertension related to the results of single factor analysis in multivariate analysis of Logistic regression model,Through the Logistic regression analysis showed that mainly animal oil(OR=1.198,95%CI: 1.423,1.671),pessimism(OR=1.689,95%CI:1.512,2.022),family history of cardiovascular disease(OR=2.69,95%CI:1.259,2.085),family history of hypertension(OR=5.74,95%CI:2.583,5.345),high blood lead content(OR=1.22,95%CI:1.327,1.621)and other factors were statistically significant(P<0.05).[Conclusions]1.There is a correlation between hypertension and lead load,the blood lead and urine lead content of the hypertension group in Lan ping lead-zinc mining area is higher than those in the control group.2.The risk factors of hypertension are genetic history of hypertension,history of cardiovascular and cerebrovascular diseases,pessimism,high blood lead content,mainly animal oil.
