Application Analysis Of Nasal Septal Cartilage In Short Nasal Deformity Rhinoplasty

Objectives the current rhinoplasty has been developed from the simple rhinoplasty to the comprehensive rhinoplasty.The simple rhinoplasty only can heighten the nasal back,the result of improving the nose tip shape is not very satisfactory.with the improvement of people's requirements,the satisfaction rate is becoming lower and lower.Yet as time passed,the long-term stress on the top of the nasal tip of the skin,making the skin of the nose tip becomes thinner,the complication incidence rate such as the implant exposure is high.The comprehensive rhinoplasty is a good surgical method which can solve a series of problems at the same time,such as the low nasal dorsum,the low nasal tip projection,the short nose,the hump nose,the broad nose deformity,the shape of the nostrils and so on.In all serious conditions,the short nose deformity is a very difficult problem to deal with.This subject mainly research the clinical effect of using the autogenous septal cartilage extend transplantation to prolong the nasal length and improve the excessive nasolabial Angle.Methods To measure the preoperative values of all cases included in our research,which include the nose length,the ideal nose length,the projection of the nasal,the nasolabial angle,the nasofrontal angle,the length of the mid-face,and the length of the underface.Taking personalized operation scheme for different patients.Surgery using open incision,to retain the nasal septum cartilage dorsal and tail side of the 1cm wide cartilage after cutting the nasal septal cartilage.Cut off the nasal septal cartilage appropriate cutting,line side of the nasal septum side of the extension of the side,to take fixed-suspended suture fixation method,the contralateral use of distraction transplantation,two pieces of cartilage in the nose at the junction of suture fixation.Intraoperative access to the nasal septal cartilage smaller and then auxiliary with the ear cartilage,prosthesis carved into a willow-shaped,nose no part of the prosthesis.Surgery combined use of cartilage,skin and soft tissue treatment to extend the nose long,in order to achieve satisfactory surgical results.The cases which just used nasal septum cartilage were divided into group A,and the cases which make a combination of nasal septum and ear cartilage were divided into group B.Measure the values which include after 6 months of the nose length,the ideal nose length,the projection of the nasal,the nasolabial angle,and the nasofrontal angle respectively,and use statistical analysis of variance analysis.Results The average extended length of the nose is 4.55 mm,the extended ideal nose length is 2.85 mm,the top projiection is increased by 2.66 mm,the frontal nasal angle increases by 8.26 degrees,and the nasolabial angle decreases by 7.57 degrees.The values of nasal length,ideal nasal length,nasal tip protrusion,nasal frontal angle,nasolabial angle and so on were compared with those of group A and group B,and the P value was less than0.05,which was statistically significant.A,B between the two groups before and after the difference between the value of the P value was greater than 0.05,not statistically significant.All the 36 cases were healed after surgery,1 case of postoperative nasal prosthesis left,3 weeks after surgery to repair surgery,1 case due to nasal septal cartilage is too small,although the effect is improved but not ideal,but patients Refused to autologous rib cartilage graft,no treatment.Conclusions The nasal septal cartilage transplantation is an effective surgical method to correct the deformity of short nose,the operation is not complicated,and is also proved to be safe and effective,it can effectively improve the nasal tip shape,make the nasal appearance at or close to the human nasal aesthetic standard.In order to ensure the operation effect,we suggest to use the autogenous costal cartilage for some people who have early known the turth that they have no enough nasal septal cartilage.Using the cartilage to replace prosthesis lying on the nasal tip can effectively reduce the complications such as the skin thinning of the tip of the nose,the implant exposure and so on,it can increse the postoperative long-term safety coefficient.
