Evaluation On Glucolipid Metabolism Of Dietary Nutrition Intervention To Patients Wih Type 2 Diabetes Mellitus

Objective:To explore the therapeutic effect and clinical significance of dietary guidance to patients with type 2 diabetes mellitus which blood glucose control was substandard.Methods:125 cases of patient with type 2 diabetes mellitus were randomly divided into control group(n=65)and observation group(n=60).The control group went to see endocrinologists as usual,while the observation group went to see nutritionists to be given dietary guidance at the same time.Patients in observation group were given professional and individual dietary guidance according to the dietary habit,physical activity,level of blood glucose and blood lipid.The follow-up time was 3 months.Blood glucose,glycosylated hemoglobin A1 c,blood lipid,body weight and waist circumference were monitored.Results:In the observation group,the level of body weight,waist circumference,total cholesterol(TC),low desity lipoprotein(LDL),free fatty acid(FFA),glycosylated hemoglobin A1c(Hb A1c),fasting plasma glucose(FBG)and 2 hours postprandial blood glucose(2h PBG)were significantly lower after treatment than before(t=3.464~7.492,P<0.001).The level of high desity lipoprotein(HDL)were significantly higher after treatment than berore(t=-3.51,P<0.001).There was no statistical difference in the level of triglycerides(TG)before and after treatment(t=1.736,P>0.05).After treatment there was no statistical difference in the levelof body weight,waist circumference and TG in the control group(t=0.635~1.248,P>0.05).The level of TC,LDL,FFA,Hb A1 c,FBG and2 h PBG were significantly lower after treatment than berore in the control group(t=2.751~6.791,P<0.001).In the control group the level of high desity lipoproteinwere significantly higher after treatment than berore(t=-3.322,P<0.001).In the observation group,the degree of body weight,waist circumference,TC and LDL decrease was higher than that of control group(t=2.697~3.284,P<0.05).There was no statistical differencein the level of TG,HDL,FFA,Hb A1 c,FBG and 2h PBGbefore and after treatment between two groups(t=-0.089?0.04~1.464,P>0.05).After treatment,the ratio of Hb A1c(<6.5%)and FBG(< 7mmol/L)in observation group was respectivelymuch higher than thatin control group(c2=5.928,4.147,P<0.05).There was no statistical difference in the ratio of 2h PBG(<10 mmol/L)in the two groups after treatment(c2=2.711,P>0.05).Conclusion:Dietary guidance joint with nutrition clinic could make further improvement in the level of Hb A1c?FBG?2h PBG?TC?HDL?LDL?FFA of the patients with type2 diabetes mellitus which blood glucose control was substandard.Furthermore,the standard ratio of Hb A1c?FBG was enhanced.It was helpful to control body weight and improve abdominal obesity.
