Background:The number of adults with type 2 diabetes mellitus in China has reached]16.4 million.Type 2 diabetes mellitus is closely related to obesity.On the one hand,being overweight or obese can lead to poor blood glucose control,on the other hand,it is also a risk factor for metabolic diseases such as cardiovascular and cerebrovascular diseases,metabolic syndrome,and diabetes-related complications.As a diet therapy for diabetes mellitus prevention and control,it has a good effect on stabilizing blood sugar and regulating posture.Compared with general diet therapy,traditional Chinese medicine diet emphasizes the four kinds of feature and five flavors of food,and emphasizes the differentiation of syndromes and syndromes,which can play the role of combining medicine with food.Wentang Wotou is a high-fibre dietary staple food developed under the guidance of Chinese medicine theory and combining the knowledge of dietetics,nutrition and food science.Therefore,this study not only helps to improve the blood glucose and obesity of patients with type 2 diabetes mellitus,but also adds a new staple food option for patients with type 2 diabetes mellitus under strict diet controlObjective:To observe the effect of Wantang Wotou on blood glucose,somatotype-related indicators and TCM syndrome scores of patients with type 2 diabetes mellitus,to study the effect of Wantang Wotou on human metabolism,and to initially understand the clinical efficacy of Wantang Wotou in the intervention of type 2 diabetes mellitusMethods:This study is a prospective,self-control,and intervention study.The study population was type 2 diabetes mellitus patients mainly from Beijing.The intervention method is to replace the main meal of a single meal with the main meal of the medicinal diet,the meal order was chosen by paitients themselves,and maintain the initial diet,exercise,drugs and other programs unchanged.The duration of the intervention was 14 days,which was divided into introduction period and observation period.The mainstay of the use of medicinal food is called "Wantang Wotou",a kind of frozen products were provided by Beijing Xinji Weiye Food Co.,Ltd.The edible dosage is designed according to individuality,and the daily carbohydrates account for 45%of the daily energy requirement.Calculate the grams of carbohydrates in the main meal of a single meal based on three meals a day,and replace it with the number of Wentang Wotou.Observe the changes in indicators before and after the intervention.E valuation indicators are divided into curative effect indicators and safety indicators.The curative effect indexes are subdivided into body type related indexes,blood glucose related indexes and TCM syndrome information.Body type related indexes include body weight,body mass index(BMI),body fat rate,waist circumference and visceral fat index;blood glucose related indexes include fasting Blood glucose,postprandial blood glucose,glycated serum protein,fasting insulin,insulin resistance index and insulin sensitivity index;TCM syndrome points are divided into the inner deficiency syndromes and the syndromes of External actual.The syndromes of inner deficiency include Yin deficiency syndrome,Yang deficiency syndrome,Qi-deficiency syndrome and blood-deficiency syndrome,the external actual syndromes include qi depression syndrome,blood stasis syndrome,liver-yang hyperactivity syndrome,phlegm-dampness syndrome,heat-gathered syndrome,depressed-heat syndrome,damp-heat syndrome.Safety indicators include blood pressure,blood lipids,liver function,and kidney function,among which blood lipids include total cholesterol,triglycerides,high-density lipoprotein cholesterol,and low-density lipoprotein cholesterol;liver functions include alanine aminotransferase,aspartic acid Aminotransferase;renal function contains urea nitrogen,creatinine,uric acid.IBM Statistics SPSS25.0 software was used to perform statistical processing on the collected data,with P<0.05 as statistical significance and P<0.01 as significant statistical significanceResults:The study conducted from February 2019 to January 2020.(1)Analysis of general enrollment:46 patients met the enrollment criteria,including 42 outpatients and 4 inpatients.Of the 46 patients,39 patients completed the study,7 were dropped,1 was eliminated,and the total number of effective samples was 38.Among them,15 were males and 23 were females.The overall average age was 56.79 years old,and patients over 51 years old accounted for 76.31%.The current medical history and past history were mainly due to diabetes mellitus complications,hypertension,and hyperlipidemia(2)Analysis of curative effect indicators:The results of self-control before and after showed that after diet intervention with Wentangwotou:? Results of somatotype-related indicators:weight decreased 0.43kg(P=0.008),BMI decreased 0.15kg/m2(P=0.01),The waist circumference of men decreased by 2.18 cm(P=0.002),and the waist circumference of women decreased by 2.5 cm(P=0.002);?results of blood glucose-related indicators:fasting blood glucose decreased by 0.24 mmol/L(P=0.021);? TCM syndrome score:It decreased that the symptom scores of which include Yin deficiency Syndrome(P=0.000),Yang deficiency syndrome(P=0.000),Qi deficiency syndrome(P=0.001),Qi depression syndrome(P=0.003),blood stasis syndrome(P =0.007),liver-yang hyperactivity syndrome(P=0.001),phlegm and dampness syndrome(P= 0.01 1),heat-gathered syndrome(P=0.028),depressed-heat syndrome(P=0.033),damp-heat syndrome(P=0.001).There was no statistical difference between other indicators before and after intervention(3)Safety index analysis:Alanine aminotransferase increased by 1.05 U/L(P=0.048)There was no statistical difference between other indicators before and after intervention.Conclusion:Replacing the one-day meal staples of type 2 diabetic patients with Wentang Wotou can significantly reduce weight and waist circumference and BMI,at the same time,and also have the effect of lowering fasting blood glucose.
