Atrial fibrillation(AF)is the most severe and most common rapid atrial electrical activity disorder in clinical.According to statistics,prevalence rate is as high as 0.08%-0.08% in today's world population,and the prevalence and appear gradually increased along with the age growth trend.When in atrial fibrillation,atrial fibrillation due to the loss of effective contraction rhythm,which led to the decline or loss of pump function,blood loss can be reduced by up to 25%.With the increase of time,atrial fibrosis,starch deposition,atrial structural remodeling and then atrial enlargement appeared.With atrial enlargement and blood sedimentation,local vortex formation,the injury of endothelial cells,stimulate the coagulation mechanism of a series,and then promote thrombosis,embolism of left atrial thrombus once the cycle to lead off,especially ischemic stroke is one of the most serious complications of atrial fibrillation embolism,its morbidity and mortality is high.It has been clearly studied that according to statistics,nearly 20% of ischemic stroke is caused by the cardiac embolus detachment.A number of studies have confirmed that left atrial thrombosis is closely related to the occurrence of ischemic stroke,and The size of the left atrial diameter is closely related to thrombosis.2012 the European Society of Cardiology guidelines strongly recommended that CHA2DS2-VASc as ischemic stroke in patients with atrial fibrillation risk assessment.but its predicted that there will be limitations,and the value in a secondary position.A large number of studies have found that there is some blood biochemical indexes and left atrium diameter can evaluate the risk of ischemic cerebral apoplexy,joint CHA2DS2-VASc score as a new development trend for stroke risk stratification,further master stroke caused by other risk factors,for the prevention of ischemic cerebral apoplexy and reasonable clinical thought more system is put forward.Therefore,this article is concerned with the risk factors of atrial fibrillation associated with stroke,focusing on the analysis of the relationship between the left atrial diameter and other clinical risk factors of stroke.Objective:Investigate the relevant risk factors of stroke in patients with atrial fibrillation,to provide effective preventive measures for the prevention of ischemic stroke caused by atrial fibrillation.Methods:A total of 136 with atrial fibrillation in the department of cardiology and neurology were enrolled,from September 2015 to December 2016,divided into two groups: 66 patients A group(without stroke group),70 patients B group(stroke group).Statisticsed the date of two groups in general information: age,gender,history of hypertension and diabetes,heart failure,the typ of atrial fibrillation;auxiliary examination data: echocardiography,blood biochemical index,thyroid function.The general condition of two groups were analyzed by single factor analysis,The significant single factors were analyzed by logistic regression analysis to investigate the risk factors of atrial fibrillation complicated with ischemic stroke.Results:1 General data analysis showed that the hypertension,gender and age of patients with stroke were significantly higher than those of non stroke group,the difference was statistically significant(P < 0.05).2 Comparative analysis of blood biochemical indexes and thyroid function: glycerin three fat,low density lipoprotein,total cholesterol,thyroid stimulating hormone,homocysteine,and stroke group was significantly higher than non stroke group and non stroke group;free thyroxine levels were significantly lower than those of the stroke group,the difference was statistically significant(P < 0.05).3 There was a significant difference between the two groups of echocardiographic index of left atrium diameter(P < 0.05),combined The stroke group was significantly higher than that of the without stroke group.4 The differences statistically significant risk indicators into the multi-factor logistic regression analysis it is concluded that: left atrium diameter greater than 42 mm,is a history of high blood pressure,total cholesterol leels were heart atrial fibrillation combined independent risk factor for ischemic stroke.5 The contrast af type similar between the two groups has no statistical significance(P > 0.05).Conclusions:1 General information in the history of high blood pressure is atrial fibrillation combined independent risk factor for stroke,active prevention and treatment of the above factors and formulate reasonable solutions to better prevent the occurrence of cerebral apoplexy.2 Echocardiographic index increased left atrial diameter is the risk factor of ischemic stroke,should give positive clinical intervention to prevent nausea events occur.3 Type has no obvious relation with the incidence of ischemic cerebral apoplexy.4 Blood biochemical indexes,elevated levels of total cholesterol is a risk factor for ischemic stroke.5 TSH and FT4,significant differences comparing the two groups,the occurrence of ischemic stroke in a certain extent may have relationship with FT4 and TSH.
