Objective:Malnutrition is one of the most common complications of ulcerative colitis,and poor nutrition can lead to imbalance of intestinal flora,intestinal mucosal immune disorder,disease progression and even worse.Assess nutritional status in patients with ulcerative colitis,and develop accurate nutrition therapy is very important.Our study used laboratory index,PG-SGA scale,NRS2002 scale and MNA scale of nutrition status and nutrition survey of hospitalized patients with ulcerative colitis in Xingtai People's Hospital,research the nutritional status of patients with ulcerative colitis.Methods : Selecting of inpatients in Xingtai People's Hospital from November 2014 to October 2016,95 patients with ulcerative colitis are selected,and the patients who met the inclusion criteria of nutritional status and nutritional support of randomized controlled trials.The survey included the basic information of patients,clinical data,laboratory index,nutritional evaluation of PG-SGA,NRS 2002 nutrition risk screening and MNA mini-nutritional assessment.The MNA value will be included in the study were randomly divided into experimental group and control group,the control group was treated with conventional anti-inflammatory treatment and low residue diet,patients in the experimental group based on conventional anti-inflammatory therapy with enteral or parenteral nutrition.According to the patient's clinical condition and colonoscopy results,the treatment effect is divided into 3 grades,cured,improved and ineffective.The statistical description of the results with mean plus or minus standard deviation,the rate of measurement data and measurement data,the frequency or constituent ratio,compared with analysis of variance,t test,non parametric test X2 test or rank three conversion.An assessment of nutritional risk and laboratory results werecompared with the standard consistency test.Results:1 Diagnostic results of laboratory malnutrition.According to the malnutrition positive,the detection rate was 40%,the difference was significant(P<0.05).The laboratory examination of malnutrition in patients with 26 cases of NRS 2002 score of good nutrition,nutrition and laboratory examination there good in 9 cases of the patients NRS2002 score for malnutrition.3 PG-SGA evaluation method to evaluate the nutritional risk of malnutrition and laboratory results of 50 cases of PG-SGA scoring method of malnutrition positive,the detection rate was 52.6%,compared with no significant difference between the two methods(P > 0.05).The laboratory examination of malnutrition in patients with 8 cases of PG-SGA divided into good nutrition,and laboratory examination are of good nutrition in 3 of the patients with PG-SGA score for malnutrition.4 MNA evaluation method to evaluate the nutritional risk of malnutrition and laboratory results of 45 cases of MNA scoring method of malnutrition positive,the detection rate was 47.4%,compared with no significant difference between the two methods(P>0.05).The laboratory examination of malnutrition in patients with 16 cases of MNA score for good nutrition,and laboratory examination are of good nutrition in 6 of the patients with MNA score for malnutrition.5 Three kinds of scale and laboratory indexes were malnutrition rate of consistency,the NRS2002 scale to examine the incidence of malnutrition was40%,and the laboratory examinations of malnutrition the consistency rate is63.2%,Kappa value was 0.29,which was a poor consistency.PG-SGA scale to examine the incidence of malnutrition was 52.6%,and the indicators of laboratory examination the consistent rate of malnutrition was 88.4%,Kappa=0.77,which was an excellent agreement.MNA scale to examine the incidence of malnutrition was 47.4%,and the laboratory indexes were malnutrition rate was 76.8%,which was a highly consistent.6 Study on the relationship between disease severity and nutritional status of laboratory indicators,ulcerative colitis patients in 35 cases,including 4cases of malnutrition;40 cases of moderate ulcerative colitis patients,including 31 cases of malnutrition;severe ulcerative colitis patients in 20 cases,including 19 cases of malnutrition,severe ulcerative colitis patients malnutrition ratio was higher than that of moderate and mild group(P<0.001).7 Study on the relationship between lesion location and nutritional status of laboratory indicators,36 cases of ulcerative proctitis patients,including 15 cases of malnutrition;ulcerative colitis patients left in 37 cases,including 22 cases of malnutrition;22 cases of ulcerative colitis patients in all,including 18 cases of malnutrition,ulcerative colitis patients with malnutrition significantly above,patients with ulcerative proctitis and ulcerative colitis patients left(P<0.05).8 The patients in the experimental group and the control group were compared in general.48 cases in the experimental group: the average age was41.7±14.3 years old,male 26 cases,female 22 cases;17 cases of patients with mild ulcerative colitis,21 cases of moderate ulcerative colitis patients with severe ulcerative colitis patients in 10 cases;17 cases of ulcerative proctitis patients,19 cases of patients with left-sided colitis,12 cases of patients with colitis.The control group consisted of 47 patients with an average age of 39.8±15.5 years,27 males,20 females,and 18 patients with mild ulcerative colitis,19 cases of moderate ulcerative colitis patients,severe ulcerative colitis patients in 10 cases;19 cases of ulcerative proctitis patients,18 cases of patients with left-sided colitis,10 cases of colitis patients.There was no statistical difference between the experimental group and the controlgroup in terms of age,sex,disease severity and lesion location(P>0.05).9 Compared the experimental group and the control group after treatment of nutritional index comparison,the experimental group and control group before treatment,the laboratory indexes were no statistically significant difference(P>0.05).The patients in the experimental group after treatment,hemoglobin,albumin,total protein,serum albumin and serum were significantly increased than before treatment,after treatment of each index were statistically significant(P<0.05 patients in the control group);hemoglobin,serum total protein and egg white before compared with before treatment had no obvious change,the difference was not statistically significant(P>0.05)after treatment,serum albumin was significantly higher than before treatment(P<0.05).After treatment,the experimental group and the control group of patients with pre albumin,total protein and serum albumin were statistically significant and the difference in serum(P<0.05).10 Compared the experimental group and the control group to compare the clinical efficacy of the patients in the experimental group,33 cases were cured,12 cases improved,3 cases were healed;patients in the control group,21 cases were cured,11 cases improved,15 cases were healed.The patients in the experimental group,the cure rate and improvement rate was significantly higher than the control group,the difference was statistically significant(P<0.05).Conclusions:1 PG-SGA scale and laboratory indicators to determine the incidence of malnutrition is the same,and the consistency is good,the effect of malnutrition in patients with ulcerative colitis was significantly better than the NRS2002 scale and the MNA scale.2 Patients with severe ulcerative colitis malnutrition ratio was significantly higher than that of moderate and mild ulcerative colitis group.The malnutrition ratio was significantly higher than that of patients with ulcerative proctitis and ulcerative colitis patients left half.3 The combination of enteral nutrition and parenteral nutrition supportcan improve the malnutrition of patients,relieve the clinical symptoms effectively,and obtain better prognosis.
