Objective:To observe the onset time,duration of analgesia,motor function recovery time,block effect and the effects on cTnI and CKMB of different concentrations of ropivacaine applied to brachial plexus block under the guidance of ultrasound.Exploring the appropriate concentration of ropivacaine for ultrasound-guided axillary brachial plexus block.Methods:Sixty patients with forearm or hand fracture who were from the second hospital of Shanxi Medical University,29 males and 31 females,aged 20 to 60 years,BMI value20 to 25 kg/m2,ASA physical status ?to ?,scheduled for surgical under the repair brachial plexus block with the guidance of ultrasound were randomly divided into three groups: 0.33% ropivacaine group,0.40 % ropivacaine group,0.50% ropivacaine group.Axillary brachial plexus was blocked under the guidance of ultrasound,the onset time of analgesia,duration of analgesia,block effect,motor function recovery time and incidnce of adverse reactions were recorded.Four milliliters blood samples were taken from veins of the patients for determing the serum cTnI and CKMB before anesthesia and 24 hours after drug injection.Results:With the increase of concentration of ropivacaine,the onset time became shorter,and the duration of analgesia and motor function recovery time became longer,the difference between the groups was statistically significant(P<0.05).The effects of brachial plexus block showed no statistically significant difference between the groups.No adverse reaction occurred in three goups.The cTnI,CKMB levels increaced at 24 h after drug injection.The cTnI,CKMB levels increased with the increase of concentration of ropivacaine,and the difference between the groups was statistically significant(P<0.05).Conclusion:0.4% ropivacaine is more appropriate on ultrasound-guided brachial plexus block,because it can meet the needs of the operation,but also reduce the impact on the myocardium of patients.
