ObjectiveTo analyze the curative effect and its influencing factors for surgical treatment of Rheumatic Heart Disease with cerebral infarction.MethodsVarious clinical parameters were analyzed in 61 patients from Jan.2010 to Jan.2016 in Fujian Provincial Hospital.The 61 patients were all diagnosed as Rheumatic Heart Disease 、 cerebral infarction and treated with aortic/mitralt valve replacement and(or)mitral valvuloplasty.We evaluated the postoperative function of nervous system by using the Modified Rankin Scale.The patients were divided into 2 groups—group A whose score were 0、1、or 2 and group B whose score 3 、4 or 5.Clinical information of the patients were collected,including age,gender,AF,hypertension,diabetes,size of cerebral infarction,the time of CPB,the time for ICU,postoperative hospitalization days and postoperative follow-up etc.The data were statistically analyzed by method of single factor and multi-factors analysis.ResultsGroup A: 38 patients.Group B: 23 patients.Single factor analysis showed that AF,hypertension,diabetes,age,size of cerebral infarction,the time of CPB,the time for ICU,postoperative hospitalization days were important risk factors.Multi-factors analysis showed that AF,hypertension,size of cerebral infarction,the time of CPB were an independent risk factors.Conclusion1.Heart surgery had exact curative effect for cardiogenic cerebral infarction2.AF,hypertension,size of cerebral infarction and the time of CPB were independent risk factors for prognosis of cardiac cerebral infarction.3.Age and diabetes were important factors for prognosis of cardiac cerebral infarction.
