Objective: To analyze the correlation between the level of serum lipids and the severity of coronary artery disease in patients with coronary angiography,and to evaluate the value of coronary angiography.Methods: a retrospective analysis of cases,Affiliated Hospital of Armed Police Hospital selected from November 2013 to November 2014 with or suspected coronary heart disease patients with 1437 cases,according to coronary angiography the selected subjects were divided into coronary heart disease group(990 cases)and control group(447 cases),patients were collected clinical data and serum lipid levels,calculation of non high density lipoprotein cholesterol level and blood lipid ratio index.The degree of coronary artery disease with involvement of vessels and Gensini score.Analysis of the basic clinical data and the serum lipid and blood lipid ratio levels of the control group and the coronary heart disease group.In 1437 cases of 417 patients with blood lipid determination of Apo A1?Apo B and Lp(a),also divided into CHD group(287 cases)and control group(130 analysis of cases),coronary heart disease group and control group in Apo A1,Apo B,Apo B/ Apo A1,Lp(a)have no difference.Also analyzed whether there were differences between CHD group different coronary lesions and different Gensini integral group blood lipid level,blood lipid ratio.To analyze the relationship between risk factors,serum lipid and lipid ratio and coronary artery disease.Multivariate logistics regression analysis of risk factors for coronary heart disease.ROC was used to evaluate the diagnostic value of Lp(a)in coronary heart disease.The relationship between serum lipid levels and coronary artery lesions in patients with different levels of LDL-c.With the increase of Lp(a)level,the changes of coronary lesions were analyzed.Results: The basic clinical data of coronary heart disease group and control group were significantly increased,the difference was statistically significant(P<0.01).Comparison of coronary heart disease group and control group,the serum levels of blood lipid and blood lipid ratio,there were significantly higher in CHD group than control group(P<0.01).Comparison of different coronary lesions in coronary heart disease group include TC,TG,LDL-c,Non-HDL-c,TC/ HDL-c,TG/ HDL-c,LDL-c/HDL-c,Apo B Apo B/,Apo A1,Lp(a)level were gradually increased(P<0.01).Comparison of different Gensini scores between groups of CHD group blood lipid levels and blood lipid ratio were gradually increased with the increase of Gensini score(P<0.01).Multivariate logistic regression analysis showed that Lp(a)was independent risk factors for coronary heart disease(P<0.05).The area under the ROC curve of Lp(a)was 0.594,suggesting a predictive significance for coronary heart disease.When LDL-c < 2.6mmol/L,only Lp(a)is associated with coronary Gensini score;when LDL-c is larger than 2.6mmol/L,LDL-c,non-HDL-c,LDL-c/HDL-c,Apo B,Apo B / Apo A1,Lp(a)were independently associated with coronary Gensini score.With Lp(a)gradually increased,the number of coronary artery stenosis and Gensini score increased.Conclusion: 1? Gender,age,hypertension,diabetes,smoking and coronary heart disease.2? The coronary heart disease group serum TC,TG,LDL-c,Non-HDL-c,TC/ HDL-c,TG/ HDL-c,LDL-c/ HDL-c,Apo B Apo B/,Apo A1,LP(a)level was significantly higher than the control group,there was may be atherogenic.3? The blood lipid in the second of CHD group gradually increased with the increase of the number of diseased coronary artery,gradually increased with the increase of Gensini integral.4? Only LP(a)and coronary artery lesions in the blood lipid factors related to the prediction of coronary heart disease.5? LDL-c layered analysis,when the LDL-c level is in the ideal level when only LP(a)for the factors related to the degree of coronary artery disease,when LDL-c is higher than the ideal level,LDL-c,non-HDL-c,LDL-c/HDL-c,TC/,HDL-c,Apo B / Apo A1,Lp(a)were the independent risk factors of coronary heart disease,coronary artery disease is a variety of tips factors,regardless of whether the increased lipid Lp(a)is caused by coronary artery disease risk factors.With the increase of LDL-c level.6? LP(a)gradually increased the severity of coronary artery damage.
