| Objective: to study the therapeutic effect of Huang qi liu wei sheng mai Decoction on the proteinuria in patients with diabetic nephropathy IV phase characterizing deficiency of both Qi and Yin.Methods:1.According to case inclusion criteria,60 patients with diabetic nephropathy and characterizing deficiency of both Qi and Yin were randomly divided into the treatment group and the control group,30 cases in each group.2.Medication method:(1)control group: Reasonable control of blood pressure、blood glucose、blood lipid and symptomatic treatment is the basis.At the same time,the 30 cases must take orally Irbesartan Tablets 150mg/day.(2)Treatment group: On the basis of treating the control group,treatment group need additional oral traditional Chinese medicine of Huang qi liu wei sheng mai Decoction,3 times/day.3.Treatment: 4 weeks for a course of treatment,a total of 2 courses of treatment were observed.4.The proteinuria,24 hour urine protein,urinary albumin,TCM syndrome score,renal function and related safety indicators of the two groups before and after treatment were statistically compared,so as to evaluate the efficacy and safety of the treatment.Results:1.Comparison of clinical efficacy: the total effective rate in the treatment group was 83.33%,and the total effective rate in the control group was 63.33%.There was a significant difference between the two groups(P=0.020<0.05).It indicate that after treatment,the total effective rate of the treatment group was better than that of the control group after t treatment.2.Comparison of TCM symptom curative effect: the total effective rate of the treatment group was 90.00%,and the total effective rate of the control group was66.67%.The results of the two groups were statistically significant(P=0.007<0.05).It showed that after treatment,the improvement of TCM symptoms in the treatment group is obviously better than that of the control group.3.Comparison of TCM syndrome scores: The results in the treatment group and the control group were all P=0.000,P<0.01,and the comparison between the treatment group and the control group was P=0.000(P<0.01).It indicated that after treatment,the TCM symptom scores of the treatment group and the control group were lower than before treatment.And the TCM symptom scores of the treatment group was significantly lower than that of the control group.4.Comparison of urinary protein:(1)The comparison of the curative effect of urinary protein,the total effective rate of the treatment group was 86.67%,while the total effective rate of the control group was 70%.There was a significant difference between the two groups(P=0.025<0.05).It showed that the treatment group was better than the control group in reducing the curative effect of urine protein.(2)After treatment,the urine protein qualitative,24 hour urine protein quantitative and urine microalbuminuria were statistically significant(P<0.01)in the treatment group,the difference was statistically significant in the control group(P<0.05).There was a statistically significant difference between the groups(P<0.05).It indicated that after treatment,the proteinuria,24 hours urine protein and microalbuminuria were all reduced,and the treatment group was significantly better than the control group.5.Comparison of renal function: after treatment,the blood urea nitrogen,serum creatinine,creatinine clearance rate in the treatment group was statistically significant(P<0.01),the difference was statistically significant in the control group(P<0.05).There was a statistically significant difference between the groups (P<0.05).The results showed that after treatment,both BUN and Scr were reduced,and Ccr was increased.The effect of the treatment group was obviously better than that of the control group.6.Comparison of safety indexes: treatment group and control group in the liver function,blood routine,urine routine,stool routine,electrocardiogram,renal ultrasound,blood lipid showed no statistical significance(P>0.05).It shows that the drugs have no obvious effect on these safety indexes,and there is no adverse reaction in the two groups of patients during the treatment.The drug is safe to take.Conclusion:1.In the treatment of diabetic nephropathy IV phase characterizing deficiency of both Qi and Yin,the total effective rate of treatment group(Huang qi liu wei sheng mai Decoction combined with western medicine routine treatment)is significantly higher than that of control group(simple routine treatment of western Medicine).2.In treatment of TCM symptoms in patients with diabetic nephropathy IV phase characterizing deficiency of both Qi and Yin,treatment group(Huang qi liu wei sheng mai Decoction combined with western medicine routine treatment)than the control group(simple routine treatment of western Medicine)can significantly improve the TCM symptoms of patients,and improve the patient’s quality of daily life.3.In treatment of urinary protein in patients with diabetic nephropathy IV phase characterizing deficiency of both Qi and Yin,treatment group(Huang qi liu wei sheng mai Decoction combined with western medicine routine treatment)than the control group(simple routine treatment of western Medicine)can significantly reduce the urinary protein,24 hour urinary protein and microalbuminuria.4.In treatment of renal function in patients with diabetic nephropathy IV phase characterizing deficiency of both Qi and Yin,the treatment group(Huang qi liu wei sheng mai Decoction combined with western medicine routine treatment)than the control group(simple routine treatment of western Medicine)can significantly reduce the blood urea nitrogen,serum creatinine,and increased creatinine clearance rate significantly,and improve renal function of patients.5.In the study,the treatment group(Huang qi liu wei sheng mai Decoction combined with western medicine routine treatment)had significant clinical effect,and no adverse reactions occurred during the treatment,which is worthy of further promotion. |
