Objective To observe the different influence of the structure and fu nction after storage in vitro through autologous red cell separation technology,and the different influence of Physiological Indexes with different erythrocyte collection.To evaluate the safety and effectiveness of pre-operative storage of autologous blood transfusion in clinical application.Methods To elective 60 cases of elective surgery,ASA I to grade II,age3060 years,according to the weight of the patient,the total blood volume?TBV?was calculated within 10%of the patient's blood,retransfusion i n vitro?4??after storage for 1 weeks.According to the patients of the level of TBV,all were randomly divided into 3groups:Group I:storage of autologous whole blood400ml;Group II:Storage of red blood cells in the whole blood of 400ml;Group III:Storage of red blood cells in the whole blood of 600ml;To observe the diffent changes of the structure and function of RBC in group I and group II:Ultrastructure of erythrocyte under electron microscope,hemolytic ratio,Oxygen release function of red blood ce ll?ATP enzyme,2,3-DPG?,Adenosine Triphosphate;To monitor hemodynamic indexes,arterial blood gas index,electrolyte,blood routine and coagulation function in group I,II,III through transfusion on the 6 Points,eg:Before blood collection?T0?,immediately after blood collection?T1?,before transfusion?T2?,after transfusion 24h?T3?,48h?T4?and 72h?T5?.Results There was no significant difference in sex,age,weight and operation time between the 4 groups?P>0.05?,after 1 weeks of blood storage in vitro,Under scanning electron microscope,red cells of I group and II group appeared rough surface and spinous protrusion.Under transmission electron microscope,there was no significant difference in erythrocyte density between I group and II group..The ac tivity of Na+-K+-ATP enzyme and Ca2+-ATP enzyme?ATPin the two groups of RBC had no significant difference?P>0.05?;The activity of Na+-K+-ATP enzyme and Ca2+-ATP enzyme in two groups of RBC,ATP content,2,3-DPG concentration and hemolysis rate were not statistically significant;The MAP and CVP in gr oup IV decreased more in T1than in T0,and obviously lower than that of I,II and III groups?P<0.05?.The values of Hb,HCT and Pa O 2 of the 3groups in T3,T4 and T5 were significantly higher than those of T2.Group I was injected with allogeneic blood 1050 ml,group II was allogeneic blood transfusion 935ml,III group transfusion allogeneic blood 450ml.Conclusion Red blood cell separation of whole blood were collected before surgery,after a week of storage,compared with traditional autologous blood storage method,has no obvious influence on the structure and function of red blood cells;after the infusion of the patients physiological index has no significant adverse effects;before the operation in good condition were collected 400600ml blood or pure r ed cell storage are safe and feasible,to protect the good effect of blood.
