A Case Study On The Case Management Of H In X Community

It is the responsibility of the social work to pay attention to and solve the problems of the elderly and improve their confidence in their life,who face the problems of physical debility,lack of communication,the lack of social intercourse,the psychological anxiety and emotional weakness caused by many factors.Based on the general system theory and social support theory,this study uses the service model of case management to interfere with the life and emotional problems of the elderly in the community.In this project,social workers,in accordance with the professional case management methods of accepting cases,predicting problems,defining problems,formulating service transmission plans,concluding cases,and evaluating cases,use the case management model based on objectives and responsibilities,and link the complicated case management right from the surrounding communities,governments and help the society to solve cases.To solve:(1)in the face of unreasonable emotional distress of the case owner,social workers use rational emotional therapy to dispel their irrational belief,guide the case owner to establish rational belief,and play a positive role in guiding the case owner to face life.(2)in view of the family life difficulties of the case owner,such as physical discomfort and heavy housework,social workers help the case owner to link informal support systems,such as neighbors and volunteers,according to social support theories,and solve their daily difficulties.(3)in view of the problem of poor health and risk of loss,social workers through the link to the Zhengzhou Charity Federation and other formal support system,to apply the Yellow hand ring and install the "wisdom community" crisis interception system in the home and solved the unexpected concerns of the case owner.(4)in view of the economic difficulties caused by life and lack of dependence,social workers help the relevant organizations to handle the retirement procedures for the son of the case owner and raise their income level.(5)in view of the fact that the case owner has no old-age security,social workers help them connect to the real estate agency and the pension institution,and improve the living conditions by means of "entering the institution" and "housing for the aged",and try to explore the application mechanism,means and methods of case management in the old social work.
