Yinchenhao decoction is a typical recipe of Zhang Zhongjing,a medical saint,and it is first recorded in Treatise on Febrile Diseases and Synopsis of Golden Chamber.Yinchenhao decoction,as a specialized recipe for the treatment of jaundice with damp-heat pathogen,is characterized by strictly-selected components,precise and simple use of medications,and marked efficacy,so it is promoted by doctors of successive generations,and its modern clinical application is very wide as well.The author of this paper summarized the use of this recipe by ancient doctors and modern clinicians through literature study methods and data mining techniques to provide guidance for TCM clinical practice.The paper consists of four parts:In terms of literature review,the clinical reports and the results of latest experimental studies about Yinchenhao decoction during the recent decades were collected and summarized to have a mastery of its indications and application range in the modern clinical practice and comprehensively learn the study progress and status quo of this recipe for the purpose of providing ideas and references for its clinical application and providing directions for the future scientific studies.The studies on Theory of Prescriptions of Yinchenhao decoction are carried out in six sections.1.Determine the source of Yinchenhao decoction and analyze the related articles.Article 236 of Treatise on Febrile Diseases expounds the etiologic factors and pathogenesis of Yangming diseases:hygropyretic jaundice with the syndrome of the steaming of damp heat,and Article 260 mainly discusses the main points in differentiation of symptoms and signs and the typical symptoms of hygropyretic jaundice."The body is orange-yellow" describes the typical clinical manifestations of hygropyretic jaundice,but it is not the most important point in differentiation.Article 259,"after the promotion of sweating in cold damage,there is a yellow color in the eyes":although what is described is cold-damp jaudice,it also points out that the symptom of jaundice is "yellow eyes",so hygropyretic jaundice patients should present with yellow body,yellow eyes,and yellow urine,and the color of body is orange-yellow.It is described in "Treatment of abnormal pulse syndromes based on syndrome differentiation Article 21" of Treatise on Febrile Diseases that,"In Yangming disease,fever and sweating indicate that heat has gone astray,and this prevents yellowish.Sweating only on the head,with no generalized sweating or sweating on the neck,inhibited urination,and thirst when taking water indicate stagnated interior heat.Generalized yellowing will occur.It has the effects of "purgation".Yinchenhao decoction is indicated",which is basically the same with Article 236,but it additionally states "the effects of purgation"and points out the recipe has the efficacy of promoting defecation.2.The studies on Theory of Prescriptions of Yinchenhao decoction.The author,through the analysis and study on the related articles and previous literatures,concludes that,the indications of the recipe lie in spleen,stomach,liver and gallbladder,the pathogenesis is hygropyretic jaundice mainly due to the affection by exopathogens such as damp heat,epidemic toxin,evil of cold dampness,internal injuries,improper diet,overtiredness or recurrence,and its pathological factors include damp evil,heat evil,cold evil,epidemic toxin,stagnation of qi and blood stasis.Its clinical symptoms include "sweating only on the head,no generalized sweating or sweating on the neck","inhibited urination","thirst when taking water","the body is orange-yellow","mild abdominal bulging" and "discomfort in heart and chest".These clinical manifestations reflect the pathogenesis features of its indications:the damp heat accumulates at the triple warmers,and becomes steamed and yellowish.3.Summarize the yellowish of different patterns of syndromes in Treatise on Febrile Diseases and Synopsis of Golden Chamber to carry out the precise differentiation of symptoms and signs and give the correct treatment.The yellowish diseases involved in Treatise on Febrile Diseases and Synopsis of Golden Chamber include multiple types:hygropyretic jaundice,cold-damp jaudice,Shaoyang acupoint jaudice,exterior depression jaudice,exterior deficiency jaudice,jaudice induced by wrongly use of cautery puncture,jaudice induced by consumptive disease,jaundice due to accumulation of stagnant blood and jaundice caused by fluid-consuming constipation.4.Make a comparison with Zhang Zhongjing's other jaudice-treating recipes to find the similarities and differences of all recipes and learn the corresponding use.The recipes recorded in Treatise on Febrile Diseases and Synopsis of Golden Chamber to treat jaudice include Yinchenhao decoction,Zhizibopi decoction,Mahuang lianqiao chixiaodou decoction,Zhizi Dahuang decoction,Yinchen wuling powder,Dahuang xiaoshi decoction,Xiaoshi fanshi powder,Guizhi huangqi decoction,Xiaojianzhong decoction,Xiaochaihu decoction,Zhugaofajian,and Guadi decoction.The above recipes can be classified into two types:1.Yinchenhao decoction and its similar recipes;2.Other jaundce-treating recipes.Yinchenhao decoction and its similar recipes include Zhizibopi decoction,Mahuang lianqiao chixiaodou decoction,Zhizi Dahuang decoction,and Yinchen wuling powder,the first two of which are from Treatise on Febrile Diseases and the last three are from "Jaundice" of Synopsis of Golden Chamber.5.Discuss the modification and development of subsequent doctors.The author of this chapter adopted the software of "Zhonghua Yidian" standalone version V5.0 and TCM ancient book databases in www.tcm100.com and found 102 derived prescriptions of Yinchenhao decoction to make a longitudinal analysis of the regularities of the application of Yinchenhao decoction in successive dynasties.In terms of the study of modern medical cases of Yinchenhao decoction,the clinical cases of Yinchenhao decoction during the recent 60 years were collected,and the SPSS 18.0 statistical software was used to establish the corresponding databases,study the information of disease of diseases in both TCM and western medicine,symptoms,picture of the tongue,pulse tracings,dose and flavor-adding drugs and make a statistical analysis on the frequency and related proportion,for the purpose of further investigating the indication characteristics and the application law of Yinchenhao decoction and further improving the clinical efficacy.Through the statistical analysis,it has been found that,this recipe is clinically applied very widely,involving multiple diseases in different departments,but the dominating diseases are liver and gallbladder diseases,and specifically the acute icteric hepatitis and viral hepatitis in the digestive system.The indications of this recipe in the clinical application are summarized:the symptoms of deep-colored urine,yellowish body and eyes,constipation,anorexia,weakness,nausea and vomiting,red or dark red tongue proper,yellow greasy or yellow thick and greasy coatings of the tongue;the common doses of the constituting drugs in Yinchenhao decoction:capillary artemisia 30g;cape jasmine 10 g,15 g;rhubarb root 10 g,6 g.In terms of the Meta analysis of the clinical random control experiment on the use of Yinchenhao decoction to treat neonatal jaundice,the author of this study first carried out a systematic evaluation and Meta analysis on the clinical random control experiments of the use of Yinchenhao decoction and its modified prescribes to treat the neonatal jaundice neonatal hyperbilirubinemia published in various kinds of domestic medical journals during 1955 and February in 2016,which provides evidence-medicine related proofs for the relevant clinical medications.A total of 26 random control clinical study literatures were included in this study,a total of 2,844 patients were enrolled,and the results of Meta analysis indicated that:the comparison between the combined treatment group and the simple western medicine treatment group showed that,Yinchenhao decoction could significantly improve the overall response rate of the treatment[OR=4.35,95%CI(3.18,5.95),P<0.00001;RR=1.16,95%CI(1.13,1.20),P<0.00001;RD=0.13,95%CI(0.11,0.16),P<0.00001]and the curative ratio[OR=2.40,95%CI(1.82,3.16),P<0.00001;RR=1.52,95%CI(1.33,1.74),P<0.00001;RD=0.21,95%CI(0.15,0.27),P<0.00001],and shorten the time taken by serum total bilirubin to recover to the normal value[SMD=-1.16,95%CI(-1.33,-1.00),P<0.00001],the time taken by the jaundice to subside[SMD=-1.22,95%CI(-1.47,-0.97),P<0.00001],and the hospitalization length[SMD=-1.33,95%CI(-1.88,-0.78),P<0.00001].The analytic results could indicate that,Yinchenhao decoction can effectively treat the neonatal jaundice and significantly shorten the time taken by serum total bilirubin to recover to the normal value and the time taken by the jaundice to subside,so its efficacy was superior to that of the simple western medicine treatment.To sum up,the author gained a deeper understanding on the indications of Yinchenhao decoction through literature review,theoretical study and evidence-based medicine.On the basis of previous experiences,the author will inherit and develop it in a better way for the purpose of providing references and guidance for the modern clinical application of Yinchenhao decoction.
