Acupuncture At Jiaji Points To Intervene In The Clinical Study Of Dislocation And Resting Pain

Purpose and SignificanceSphacelus is a kind of vascular disease,its main manifestation is that it starting with limb cool extremities and numbness,developing of toe section necrotic and sloughing off later,and even becoming black,rot,and forming ulceration,and lasting sore opening as well.This subject is mainly aimed at Western medicine,lower extremity arteriosclerosis obliterans.The course of the disease is slow,and with long duration,it often increased in the cold season,and can relapse after was cured.And the main clinical performances of the disease are often suffering from limb cold,numbness,intermittent claudication,pain,arterial pulse weakened and even disappear,and even appears ulcers,and gangrene as well.With the development of the economic level,and people's living standards and habits,changes in diet as well,the incidence of sphacelus increases year by year.The poor prognosis of the disease is great harmful to human health and life.Sphacelus affects the normal life of the patient,threatens human health,and adds a heavy burden to the family and society.Rest pain is a common symptom of sphacelus,the patient suffering from torturing of resting pain,which makes a lot of patients pursuing amputation when they cannot tolerate it anymore.Rest pain does not only increase the amputation rate,but also has a negative impact on the patient's mood,what's more,long-term pain can even induce anxiety and depression,and it will further more aggravate the disease,which has an extremely negative impact on the general condition and the limb,increasing mortality.At present,the main treatment of sphacelus in Western medicine is pursuing drugs and surgery.Although expansion of blood vessels,anticoagulation,antiaglutination,analgesic and other drugs,to a certain extent,can ease the development of the disease,but for the relief of rest pain it did not satisfying,resting pain is still a trouble of sphacelus patients.Although the new means of arterial bypass,stent implantation and other solution in surgery temporarily solved the harassment in the patients with rest pain,its clinical application of large restrictions:high cost of surgery,postoperative restenosis rate is high,some elderly patients can not tolerate surgeries,or extensive vascular occlusion.Chinese medicine has more means and a greater advantage in the treatment of acupuncture resting pain compared with Western medicine,and adapt to a wide range.Therefore,from the perspective of Chinese medicine to study the pathogenesis of rest pain,and to explore the effective method of prevention and treatment of rest pain of sphacelus has become the important issues which most of clinical workers concern commonly.Research methodsThe aim of this study was to investigate the efficacy and mechanism of acupuncture on patients with rest pain at Jiaji acupoints,by randomizing,parallel-group,and non-blind clinical study.Hospitalized in Medicine Hospital of Beijing,56 patients were randomly divided into 28 experimental groups and 28 control groups,to research acupuncture on patients with resting pain at Jiaji acupoints.Taking 2 weeks as a period,NRSs were evaluated before and after the treatment,transcutaneous oxygen partial pressure and laser Doppler flowmetry were also recorded,and safety indicators was detected as well.The experimental groups were given basic and acupuncture treatment,but the control group was given some basic treatment only.Pain intensity and the NRS weighting values were used to judge the efficacy.Finally,the efficacy of relieving rest pain on patients with resting pain at Jiaji acupoints was analyzed by statistical analyzing,and the possible mechanism was explained.And this is where the innovation of this subject lies.Research results1.Subjective indicator1.1 pains:the global efficacy of experimental groups was 79.25%,the global efficacy of control groups was 55.56%.The number of patients with moderate-to-severe pain in the experimental groups decreased about 1/3 after the treatment,and some of these patients were either being moderately painful or being mildly treated after treatment,and the number of mild pains was much higher than that of the control group.1.2 extremity colors:the experimental group and the control group present no effective results.1.3 sensations of cold:the global efficacy of experimental groups was 94.34%,the global efficacy of control groups was 83.67%,the two groups have achieved significant efficacy,the experimental groups are more significant.1.4 distances of intermittent claudication:the global efficacy of experimental groups was 32.14%,the global efficacy of control groups was 10.71%,the two groups were not satisfied with the efficacy.1.5 numbness:the global efficacy of experimental groups was 94.44%,the global efficacy of control groups was 81.58%,the two groups have significant efficacy,the improvement of the experimental group is more obvious.2.Objective indicators2.1 transcutaneous oxygen partial pressures:after treatment,the mean value of the treatment group was 23.45,and that of the control group was 20.88.After the treatment,the oxygen pressure of the experimental group was increased,and the value was higher than that of the control group.2.2 laser Doppler flowmetry:the test group after treatment the average is 29.12,the control group after treatment the mean of 24.98,the experimental group than the control group increased with laser Doppler flowmetery.Conclusion1.Acupuncture at Jiaji points can improve the rest pain in patients with sphacelus,the effect is better than the basic treatment,and has significant efficacy ob improving limb numbness,and cold.2.Patients with rest pain can be alleviated by acupuncture at Jiaji points is related to the improvement of the lower limb circulation,and increase oxygen content.3.The rest pain improvement were significantly correlated with the tcpO2 improvement;was significantly related with laser Doppler flow meter improvement.SignificanceAcupuncture has a good social value and economic benefits to alleviate the rest pain,it can reduce the suffering of patients,increase cure rate,reduce treatment costs,and save social resources.
