| Background:Helicobacter pylori(H.pylori)is the only bacteria found in the human stomach so far,its main pathogenic mechanism is H.pylori can hydrolysis urea and release ammonia to cause inflammation and immune responses by injure gastric mucosal directly,but H.pylori is surrounded by ammonia clouds which caused by itself and avoid invade and attack from gastric acid and pepsin,to make it can survive in the low pH environment.Almost H.pylori infection can give rise to gastritis,more important is,continuous H.pylori infection can develop to atrophic gastritis,intestinal metaplasia(Ⅲ type)and atypical hyperplasia,finally develop to gastric cancer.In 1994,International agency of cancer research,belongs to the World Health Organization,according to the prospective epidemiological investigation data and evolvement rule of gastric cancer occurence,the H.pylori has been listed as stomach cancer’s first kind of carcinogens.H.pylori’s found is a significant revolution in the history of peptic ulcer treatment,most scholars think that H.pylori correlative ulcer must be eradicated H.pylori to reduce or prevent ulcer recurrence.H.pylori infection not only can cause gastrointestinal disease,but also can cause other diseases by the immune response,such as Iron deficiency anemia,Idiopathic thrombocytopenic purpura,Diabetes,etc.About H.pylori infection,the main therapeutic schedule are Triple therapy and Quadruple therapy.With antibiotics extensive use and nonstandard eradication program,H.pylori eradication rate is falling down,even less than ideal value 90%,also has such problems,Drug resistance,adverse reaction,recurrence,and so on.So,how to eradicate effectively and reduce the adverse reaction is the problem need to be solved.More and more Traditional Chinese medicine research in the eradication of H.pylori.Jing Hua Wei Kang capsule is representative proprietary chinese medicine,it can not only help western medicine to eradicate H.pylori,but also to improve stomach environment,and reduces the possibility of recurrence.Objective:Currently using proprietary Chinese medicine combined triple or quadruple therapy has shown good curative effect,but how to reduce application of antibiotics by using proprietary Chinese medicine still not be researched.Mentor on the basis of lng-term clinical practice,in view of some antibiotics resistance or unwilling to take large doses of antibiotics ’s patients,using Jing Hua Wei Kang capsule combined one antibiotics,proton pump inhibitors(PPI),bismuth agent had good curative effect.This research is on the basis of real world to observe new therapy’s eradication rate and safety.Research methods:Select 13C-urea breath test positive patients of Beijing university of Chinese medicine Dongzhimen hospital digestive department outpatient as the research object.Randomly divide 84 up-to-standard patients into two group,observation group 46 patients and control group 38 patients.Observation group take Bella azole 20 mg once a day,amoxicillin 1000mg two times a day,colloidal bismuth pectin 150 mg four times a day,Jing Hua Wei Kang capsule three times a day.Control group take Bella azole 20mg once a day,amoxicillin 1000mg two times a day,colloidal bismuth pectin 150mg four times a day,clarithromycin 500 mg two times a day,each 2 weeks.Treatment is given priority to outpatient service Before treatment and after the course for 4-8 weeks,review the 13C-UB breath test and record the symptoms integral.At the end of program,analyzing two groups of helicobacter pylori eradication rate and efficient,observing the symptom integral improvment and the drug adverse reactions occurred during the study,to assess the quad new therapies’H.pylori eradication efficacy and safety.Results:(1)After 2 weeks Jing Hua Wei Kang capsule quadruple new therapy,the 46 patients of helicobacter pylori infection eradication rate is 73.9%,higher than the standard quadruple therapy control group eradication rate 68.4%;(2)In the aspects of different syndrome types’ eradication rates,spleen-stomach damp-heat syndrome is hightest in two group.Second is disharmony between liver and stomach syndrome.And spleen and stomach weak syndrome patients eradication rate is the lowest,observation group is generally higher than the control group;(3)In the aspects of clinical symptoms,observation group and control group both improve significantly.the observation group is better than control group,there is statistically significant difference between the two groups;(4)During the treatment,the control group appeared 2 cases of adverse events,observation group have one adverse event.In terms of reducing adverse events,observation group is better than the control group.Conclusions:Jing Hua Wei Kang capsule combine with rabeprazole azole,amoxicillin,colloidal bismuth pectin trigeminy new antibacterial therapy can improve the eradication rate of helicobacter pylori,and effectively improve the clinical symptoms.Not only can improve the compliance of patients,relative to the standard western medicine quadruple therapy,also can reduce the incidence of adverse reactions. |
