| Aim: With the development of the society and the national economy,people’s way of life has great changes.And with the advent of the era of an aging society,the population structure in many countries have taken place great changes,changes in lifestyle and the structure of social population made the increasing number of the diabetes.Diabetes is not only a great harm to the body health,but also increased the burden of families and society.And movement as an effective intervention for diabetes,its efficacy has been gradually recognized by people.The use of sports means to control the rate of diabetes is not successful.This has become a big problem in the global public health.It has something to do with the improper choice of exercise intensity have a great relationship,and the FATmax which was put forward by Jeukendrup and Achten,can not only determine the scientific exercise intensity,but also has a great effect on improving lipid metabolism in theory,It is helpful to control cardiovascular complications,and insulin sensitivity associated with FFA,therefore,the improvement of the lipid metabolism are helpful to increase insulin sensitivity,So in theory,FATmax in patients with type 2 diabetes have improved effect.At present,many studies have confirmed that the FATmax has good effect to lipid metabolism,but the research of using FATmax intervention for elderly patients with type 2 diabetes rarely reported at home and abroad,it remains to be further research.This study was to measure FATmax Rate of the elderly female patients with type 2 diabetes through the experiment,and find out the exercise intensity and exercise heart rate,which is corresponds to the FATmax Rate,Using the exercise intensity to make exercise prescription for patients with type 2 diabetes,the experimental group performed this three-month exercise intervention with the exercise prescription.The subjects were compared before and after the experiment data to evaluate the effect of FATmax on glucolipidmetabolismin elderly women with type 2 diabetes.Methods:34 female patients with type 2 diabetes which is diagnosed by the 3 armour hospital as the object of study,Their age is between 60-69 years old.fasting glucose is greater than 7.0 mmol/ L,glucose tolerance test or random blood glucose is greater than 11.1 mmol/L,as long as satisfy any of the above two standards,can be diagnosed with diabetes.Exclusion of exercise contraindications,the subjects were divided into experimental group and control group,each group of 17 people.1.Fasting venous blood was taken before the experiment,Determination of leptin,adiponectin,lipoprotein lipase,acetyl coa dehydrogenase,acetyl coa decarboxylase,glycated albumin,cholesterol,triglycerides,high-density lipoprotein cholesterol,low density lipoprotein cholesterol and other indicators;2.simple glucose tolerance test was performed before the experiment,Extraction of fasting venous blood,75 g of glucose was orally administrated immediately(dissolved in 250-300 ml of water,and finished drinking within 5 minutes),then take2 hours of venous blood,Determination of fasting glucose,Two hours postprandial blood glucose,fasting insulin,Two hours postprandial blood insulin and other indicators;3.Test height,weight,waist circumference,hip circumference,abdominal fat percentage,visceral fat rating,body fat percentage and other indicators before the experiment;4.Use Ergoselect 100 series vertical power bicycle which is made by German and Polar heart rate table which is produced by Finland for testing,Records of each level load at the end of the heart rate and subjective motor sensory(RPE)scale to rate,according to line chart of Astrand estimated the maximum oxygen uptake(VO2max);5.Use Cortex ML 3B gas analyzer,wearing a Finnish production RS800 CX Polar Which is produced by Finland for incremental load exercise test,record Sec oxygen uptake and carbon dioxide exhalation of every 2 minutes after every 15 minutes.The numerical into the formula calculating the amount of fat oxidation which is improved by Achten [14]: 1.67×VO2-1.67×VCO2,and calculated FATmax;6.Through the experiment find out the exercise intensity and exercise heart rate,which is corresponds to the FATmax Rate,Using the exercise intensity and exercise heart rate to make exercise prescription for patients with type 2 diabetes,the experimental group performed this three-month exercise intervention with the exercise prescription.In addition,to maintain the original way of life,so that to observe the effects of maximal fat oxidation intensity through exercise training on glycolipid metabolism;7.Testing after 3 months exercise intervention with the same instrument and the same test methods.Comparing the data before and after the experiment.Results:1.The FATmax rate of elderly women patients with T2 DM is 0.37,the exercise intensity which is corresponds to the FATmax rate is 37.39±7.39%VO2max,corresponding speed was 3.59±0.60km/h,and the corresponding movement heart Rate is 100±8 p/min.Using the exercise intensity and exercise heart rate to make exercise prescription for older women with type 2 diabetes.2.Compared with before experiment,BMI of experimental group had significantly decreased(P < 0.01),Waist-to-hip ratio dropped from 0.94 to 0.94,and there was a significant difference(P < 0.01),Body fat percentage from 34.4% to 31.0%,and there are significant changes(P < 0.01),abdominal fat percentage has significantly decreased(P < 0.01),visceral fat rating decreased significantly(P < 0.01),and belongs to the normal range.While the control group there was no significant change(P>0.05).3.After 3 months of FATmax exercise intervention,triglyceride in the experimental group significantly decrease(P < 0.01),total cholesterol decreased by an average of0.36mmol/L(P < 0.01),the levels of HDL increased significantly(P < 0.01),low density lipoprotein cholesterol has a tendency of decrease(P < 0.01).Lipoprotein lipase,acetyl coa dehydrogenase and acetyl coa decarboxylase were no significant difference(P > 0.05),leptin decreased significantly(P < 0.01),adiponectin increased significantly(P < 0.01),the control group no significant change(P>0.05).4.After 3 months of FATmax exercise intervention,Fasting blood glucose in the experimental group from 7.95mmol/L to 6.99mmol/L(P<0.01),Two hours postprandial blood glucose has significantly decreased(P < 0.01),fasting insulin and two hours postprandial insulin have significantly decreased(P < 0.01),glycated serum protein decreased by an average of 30mmol/L(P < 0.01),Insulin resistance decreased significantly(P < 0.01),beta cell function no significant change(P>0.05).And insulin resistance in the control group were significantly increased(P < 0.05),the rest of the indicators there was no significant change(P>0.05).Conclusion:1.The FATmax rate of elderly women patients with T2 DM is 0.37,the exercise intensity which is corresponds to the FATmax rate is 37.39±7.39%VO2max,corresponding speed was 3.59±0.60km/h,and the corresponding movement heart Rate is 100±8 p/min.The exercise prescription formulated with this exercise intensity has a good improvement on the condition of elderly women with type 2 diabetes.2.BMI,waist hip ratio,body fat percentage,abdominal percentage and the level of visceral fat are risk factors for diabetes and its complications,through FATmax exercise can reduce the effect of these risk factors,mainly for BMI,waist hip ratio,body fat percentage,abdominal percentage and the level of visceral fat reduced.3.FATmax exercise can improve blood sugar,insulin secretion,glycated albumin and insulin resistance,suggesting FATmax exercise can improve the effect of glucolipid metabolism on elderly women with type 2 diabetes,the results provide a basis for improving the condition and controlling complications in patients with type 2diabetes. |
