| For a long time,due to the influence of the dual social structure between urban and rural areas in China,medical insurance system of urban and rural dual structure is noticeable and the fragmentation phenomenon of the system is comparatively serious.As a result,the allocation of urban and rural medical and health resources is unequal.With the development of urbanization,coordinating the urban and rural integration,narrowing the urban-rural gaps,promoting the development of integration of urban and rural medical insurance system become an important issue.In January 2016,the state council released the notice about the integration of urban and rural residents basic medical insurance system,putting forward to promote urban residents medical insurance and new rural cooperative medical system integration and establishing a unified urban and rural residents basic medical insurance system across the country according to the principle of full coverage?meeting the basic living needs?multi-level and sustainability.By the end of 2016,two-thirds of provinces have made an overall plan for establishing a unified urban and rural residents medical insurance system or the integration has been realized.The integration of urban and rural medical insurance is conducive to narrow differences between urban and rural basic medical treatment and can help to realize the fairness of medical resource allocation,which adapts to the trend of integrating the urban and rural areas.Ezou is the pilot city in Hubei Province of integrating urban and rural residents medical insurance,which has realized the integration of urban and rural residents medical insurance system since 2008 and now has passed about 10 years.During this period,the human resources and social security bureau of Ezhou have made unremitting efforts in the aspect of urban and rural residents health care that continuously innovating and improving the medical insurance management system and accumulated a wealth of experience.The research purpose of this paper is that according to investigating the residents in Ezhou who participate the urban and rural residents health care via the ways of questionnaire investigation and face-to-face interview,from the perspective of residents who participate health insurance,with the aim of residents' health care system,with the purpose of ensuring the health of the residents,through analyzing the residents'satisfactory degree to health care,building the satisfactory degree influencing factors model and analyzing the influence factors,to put forward the related suggestions to further improve the urban and rural residents health insurance in Ezhou and provide the reference for the cities that have integrated the urban residents medical insurance and new rural cooperative medical system.This paper applied the data of "urban and rural residents medical insurance satisfaction survey"of the Whuhan University social security research center,from which selected Ezhou's two cities(Fenghuang streets and Gulou streets)and two towns(Taihe town and Zhaoshan town)to study.The investigation suggested that the residents who participate the medical insurance showed satisfaction as a whole.The factors which can significantly affect the attitude of residents towards urban and rural residents medical insurance are five:the medical conditions and technical level of designated medical institution,the service attitude of medical staff,the medical insurance reimbursement ratio,the extend of reimbursement of diseases and medicine and the work ability and service attitude of the organization of management institutions.Based on the above analysis,this paper comes up with the following suggestions:in the first place,improve medical conditions and technical level,improve the quality of the medical staff.Then,establish the reasonable reimbursement ratio,keep pace with the times to adjust the scope of reimbursement.In the end,strengthen the work ability construction of medical insurance agency management institutions. |
