Clinical Study Of Wuling Powder In The Treatment Of Brain Edema Formation Following Basal Nuclei Region Hemorrhage

Purpose: To observe the clinical effect of Wuling Powder combined with mannitol injection in the treatment of patients with brain edema formation following basal nuclei region hemorrhage,it provides an objective theoretical basis which combined traditional Chinese and Western medicine become widely used in brain edema formation following cerebral hemorrhage.Method: In Guangxi traditional Chinese medicine university affiliated Ruikang hospital neurology and neurosurgery ward and the other subsidiary hospital neurology ward of KongGang from October 2015 to October 2017,collecting in conformity with the inclusion criteria of brain edema formation following basal nuclei region hemorrhage in patients with 60 cases.they were randomly divided into two groups,one treatment group and one control group(30 in each).patients in the control group were treated with routine western medicine including intravenous drip with 20% mannitol injection in q12h~q8h,while patients in the treatment group additionally accepted Wuling Powder(alisma 15g、Poria cocos 9g、grifola 9g、Atractylodes 9g、Cassia twig 6g).Both the mannitol injection of groups were continuously treated for 7~10 days,but the treatment group continue to use Wuling Powder to 20 days.During the treatment of mannitol injection dosage was depending on the the bleeding and degree of cerebral edema.Before and after 7 days and 20 days,therapeutic effect was compared according to the changes of brainimaging,renal function,electrolyte,The National Institutes of Health Stroke Scale(NIHSS),plasma osmotic pressure,fasting blood glucose and daily mean urine volume as well.Results: On the 7th day and 20 th day,the changes of cerebral edema,brain hematoma and NIHSS in the treatment group were significantly better than those in the control group(p<0.05).On the 20 th day,the NIHSS in the treatment group were significantly better than those in the control group(p<0.05).It prompted the treatment group in reducing brain hematoma and edema volume and improving nerve function recovery is better than the control group.After treatment of each observation period,the curative effect observation index including renal function,serum sodium,serum potassium,fasting plasma glucose,plasma osmotic pressure and daily average urine had no significant difference between two groups(P>0.05).Two groups of renal function,serum potassium had no significant difference within the each group in each period of time(p>0.05).Two groups of blood sodium were slightly lower at each observation time point.After treatment of 20 days,treatment group appear hyponatremia was 16% and the control group was 28%.Two groups of plasma osmotic pressure increased gradually in observation period,it has significant differences within the each group(p<0.05).In observation period two groups of fasting glucose reduced gradually,it has significant differencess within the each group(p<0.05).Conclusion: Wuling Powder with mannitol combined use of brain edema formation following basal nuclei region hemorrhage compared with pure mannitol treatment,the former can not only significantly improving brain hematoma and cerebral edema absorption,accelerating nerve functional recovery,and reducing the occurrence of electrolyte metabolism disorder,so as to significantly improve the patient’s clinical curative effect.In the treatment process of patients with brain edema formation following basal nuclei region hemorrhage,Wuling Powder had not led to adverse reactions to patients and have curative effect.It worths promoting in the clinical.
