Objective To investigate the therapeutic effect of vacuum sealing drainage in tibial exposure repaired by adjacent muscle flap and the inguinal lymph node dissection.Methods Through retrospective analysis the treatment of 97 cases with complex wound in our department from 2006 to 2016,the tibia exposed wounds in 17 cases,and malignant tumor expand excision combined with ipsilateral inguinal lymph node dissection in 80 cases.5 cases which is badly infected and tissue necrosis should perform debridemen expand combined vacuum sealing drainage before adjacent muscle flap with VSD to repair tibia exposed wounds as the others in first group;The second group were randomly divided into the VSD treatment group and the traditional pressure control group on average,daily drainage fluid volume,color,the situation of inguinal incision wound,flap survival and therapeutic effect were analyzed in the two groups.Results All of the tibial exposed wound cases cured by adjacent muscle flap combined with VSD were wound healing,as well as the 5 infected and tissue necrosis ones,resulting in controlled wound infection,negative bacterial culture,and fresh granulation.In the cases applied malignant tumor expand excision and ipsilateral inguinal lymph node dissection,34 cases(85.5%)were cured,5 cases(12.5%)were effective and 1 case(2.5%)was invalid in the VSD group,meanwhile,in the control,15(37.5%)cured,13(32.5%)effective and 12(30%)invalid.The effect of the two groups was statistically compared and the difference was statistically significant(P<0.001).Conclusion The VSD can effective drainage of wound drainage,promote microcirculation,increase the flow of blood in the wound,increase the wound ability of anti-infection and stimulate the growth of granulation tissue;It provides a new method to repair the tibial exposure wound with one time;The regional lymph node dissection of malignant tumor was combined with VSD can prevent the occurrence of lymphatic leakage and promote skin healing.
