BackgroundAutologous cartilage is widely used in plastic surgery due to its origin,convenience,good histocompatibility,no rejection,low infection and exposure rate.Cartilage transplantation,however,for the pain obviously,and possible occurall various complications include hematoma,infection,local skin necrosis,scar hyperplasia or keloid formation,nasal septum perforation of the sticky periosteal necrosis and so on,and because of cartilage defects,caused by weakened donor site under external stress,increase the area for local subsidence or cavity viscera damage risk.It is a difficult problem for the clinician to repair the defect of cartilage and the corresponding complication of wound healing.Platelet-rich fibrin(PRF)is the second generation rich platelet concentrate,it can promote angiogenesis and bone cartilage proliferation and migration and reduce inflammation reaction,is widely used in oral medicine planting.Cartilage transplantation in sculpture produced in the process of shaping cartilage debris has not been properly used,how to effectively use the elastic cartilage repair cartilage defects,improve the survival rate and promote cartilage regeneration,domestic and foreign scholars have been carrying out relevant research and exploration.In recent years,the repair of congenital or traumatic cartilage defect has been achieved by using diced cartilage alone or in combination with other biological package materials.This basic experiment design using the rich platelet fibrin(PRF)combined with diced cartilage repair rabbit ear donor site after cartilage transplantation for cartilage defect wound,in order to research large area cartilage defect repair and laid the foundation of donor site repair in clinic.ObjectiveEvaluate the effects of platelet-rich fibrin and diced cartilage on the repair of cartilage defect of donor site.To provide reference for the repair and reduction of postoperative complications of cartilage defect after clinical cartilage transplantation.Methods1.The preparation of PRF.2.To establish a model of cartilage defect in the donor area after cartilage transplantation.Take 30 New Zealand white rabbits,symmetry in each rabbit ears do 2 in area of about 1.0cm x 0.6 cm of full-thickness cartilage defects,were randomly divided into 5 groups: blank control group,diced cartilage group,PRF group,PRF+diced cartilage group and cartilage block replantation group.3.Preparation of diced cartilage.4.Corresponding treatment for each group: In the blank control group,no treatment was done in the wound surface.Autologous diced cartilage was implanted in the wound surface of diced cartilage group.PRF group was implanted with autologous PRF.PRF+diced cartilage group was implanted with PRF combined with diced cartilage complex.The cartilage block replantation group the cartilage block was grafted into the wound.The materials were collected from 4 weeks,8 weeks and 12 weeks after surgery,and were observed by macroscopic and histological observations.5.The statistical analysis of the histological repair assessment branch of each group was compared in different time period,and the repair effect was evaluated.Results1.The prepared PRF has good mechanical properties,and can be placed either in the wound or as a wrapped cartilage material.2.The animal model was successfully developed after cartilage transplantation for wound repair,and all experimental animals entered the results analysis.3.The results showed that there was no obvious cartilage regeneration in the blank control group,and the wound surface was healed at 12 weeks.Diced cartilage group wound at 4 weeks,8 weeks is scattered in the distribution of the diced cartilage filling defect,2 weeks and rim edge can be seen when the cartilage confluent,cartilage zone of central no particles into scar repair;In the PRF group,the wound surface of cartilage defect was gradually reduced,and the newborn cartilage was thinner than normal cartilage.In the 4 weeks and 8 weeks of the PRF+diced cartilage group,the diced cartilage was gradually fused with the edge of cartilage defect,and the wound surface was basically repaired at 12 weeks.The morphology is similar to normal cartilage.After the replantation of cartilage,the incisions were gradually repaired and healed at 12 weeks.4.HE staining showed that there was almost no cartilage regeneration in the blank control group,and there was a large amount of fibroblast repair defect in 12 weeks.diced cartilage group visible particle cartilage edge and cartilage defect edge near the perichondrium cartilage cell proliferation,cartilage cells of fibrous tissue between the packages,for elastic cartilage and fibrous cartilage repair;The new chondrocytes with small volume,approximate elastic cartilage cells and loose distribution were observed in the wound repair area of PRF group.All the granule cartilage grafted in the cartilage defect area of PRF+ diced cartilage were allalive,and a large number of proliferated chondrocytes were seen around,which was the regeneration and repair of elastic cartilage.cartilage block replantation group new chondrocytes can be seen in the cartilage cut edge,the cell morphology and distribution were consistent with normal elastic cartilage at 12 weeks.5.Histologic grading results through statistical analysis,according to the time,effect of PRF +diced cartilage repair group were significantly better than that of blank control group(P < 0.01),the repair effect of PRF+diced cartilage group was superior to that of diced cartilage group and PRF group(P < 0.05),the effect of diced cartilage group and the PRF group cartilage defect repair cartilage is better than that of blank control group(P < 0.05),the effect comparing with each other between in the two groups has no statistical significance(P > 0.05).Conclusion1.PRF can promote the proliferation and regeneration of rabbit ear elastic cartilage cells,which is a good biomaterial.2.PRF combined with diced cartilage transplantation can achieve the regeneration and repair of the defect cartilage,and the repair effect is better than the two alone.It is an economical,effective,safe and convenient method for the repair of cartilage defects.
