Objective:To study the significance of the detection of von Willebrand factor and D-dimer in clinical diagnosis of pulmonary embolism.Methods:1.To retrospectively analyze the clinical data of 101 patients who were hospitalized in the Third Respiratory Disease/Geriatrics ? ward of the Second Affiliated Hospital of Kunming Medical University from January 2016 to January 2018.The pulmonary embolism group was consisted of 44 patients,including 34 males and 10 females,mean age(69.22±10.24)years.The control group was consisted of 57 healthy people,including 39 males and 18 females,mean age(68.64±16.36)years.2.The following data were collected and compared between the two groups of patients:including gender,age,smoking history,related diseases,von Willebrand factor,D-dimer,fibrin(origin)degradation products,antithrombin-?,fibrinogen,platelets and other indicators for comparative analysis.Evaluate the value of the above indicators for the diagnosis of pulmonary embolism.Statistical analysis of the data was performed using SPSS 23.0 statistical software.Results:1,There was no significant difference in gender,age,age group and smoking history between the two groups(P>0.05),and they were comparable.2.There was a statistically significant difference in vWF and DD levels between the two groups(P<0.05).There was no significant difference in FDPs,FIB,AT-?,and PLT levels between the two groups(P>0.05).3.According to the normal reference values shown in the inspection items,they were divided into normal group(vWF?160%,FDPs?5ug/ml,FIB?4g/L,DD?0.5ug/ml)and abnormal group(vWF>160%,FDPs>5ug/ml,FIB>4g/L,DD>0.5ug/ml).The results showed that there was no significant difference in the incidence of pulmonary embolism between FDPs and FIB normal and abnormal groups(P>0.05);the incidence and difference of pulmonary embolism between vWF and DD normal and abnormal groups were different.Statistical significance(P<0.05).4.Selecting the best cut-off value for diagnosis of pulmonary embolism by ROC curve of vWF and DD:When vWF ?194.92%and DD ?1.24 ug/ml,the accuracy,sensitivity and negative predictive value of diagnosis of pulmonary embolism are significantly higher.For individual detection,but the specificity and positive predictive value are lower than when tested alone.Conclusion:1.vWF combined with DD has some value in the diagnosis of pulmonary embolism.2.There was no significant change in FIB,FDPs,AT-?,and PLT in pulmonary embolism.
