Objective: To explore the diagnosis values of combined detection of carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA19-9)and CA242 for colorectal cancer and distant organ metastasis.Methods: The clinical date of 86 patients diagnosed with colorectal cancer in Department of Second hospital of Dalian Medical University between July 2014 and July 2016.18 cases were colorectal cancer with distant organ metastasis,and 68 cases were colorectal cancer with no distant organ metastasis.30 patients with colorectal benign lesions diagnosised by colonoscope were be random seclected(Clotis and Colorectal polyps).All patients were diagnosed by biopsy and compare serum levels of CEA ?CA19-9 ? CA242 between the three groups.,we also compare the sensitivity,specificity,positive predictive value and negative predictive value of single detection or combined detection CEA,CA19-9,CA242 in dignosis colorectal and distant organ metastasis.Analysed the diagnosis value.All data were analyzed by SPSS 22.0 software testing.Results: 1.Serum CEA,CA19-9 and CA242 levels in the Colorectal benign lesions group were(2.89±0.25)?g/L?(8.02±4.52)KU/L?(2.51±2.665)KU/L,in the colorectal cancer with no distant organ metastasis group(9.1±8.84)?g/L ?(16.72±11.23)KU/L?(6.701±8.042)KU/L,in the colorectal cancer with distant organ metastasis group were(39±28.77)?g/L,(143.96±97.58)KU/L,(65.48±42.32)KU/L.The serum levels of CEA,CA19-9 and CA242 in colorectal cancer with distant organ metastasis patients significantly higher than to those colorectal cancer with no distant organ metastasis patients.Chi-square test: P<0.05.The serum levels of CEA,CA19-9and CA242 in colorectal cancer with no distant organ metastasis patients were significantly higher than to those colorectal benign lesions patients.Chi-square test:P<0.05.2.CEA,CA19-9 and CA242 in diagnose colorectal cancer with no distant organ metastasis:CEA: sensitivity:30.88%,specificity:86.67%,positive predictive value:86.67%,negative predictive value:35.62%.CA19-9: sensitivity: 14.71%,specificity:90 %,positive predictivevalue:76.92%,negative predictive value:31.76%.CA242: sensitivity: 8.824%,specificity:90%,positive predictive value:66.67%,negative predictive value:33.34%.CEA+CA19-9+CA242(parallel testing)in diagnose colorectal cancer with no di stant organ metastasis: sensitivity:42.65%,specificity:76.67%,positive predictive val ue:80.56%,negative predictive value:37.1%.CEA+CA19-9+CA242(series testing):sensitivity : 5.582%,specificity:100%,positive predictive value:100%,negative predictive value:31.91%.CEA,CA19-9 and CA242 in diagnose colorectal cancer with distant organ metastasis:CEA: sensitivity: 72.22%,specificity:69.12%,positive predictive value:39.39%,negative predictive value:90.38%.CA19-9: sensitivity: 66.67%,specificity: 85.29%,positive predictive value: 54.55%,negative predictive value:90.63%.CA242: sensitivity: 55.56%,specificity:91.18%,positive predictivevalue:62.5%,negative predictive value:88.57%.CEA+CA19-9: sensitivity: 77.78%,specificity:64.71%,positive redictive value: 36.84%,negative predictive value:91.67%.CEA+CA242: sensitivity: 77.78%,specificity:66.18%,positive predictive value: 37.84%,negative redictive value:91.84%.CA19-9+CA242: sensitivity: 66.67%,specificity:76.47%,positive predictive value:42.86%,negative predictive value:89.66%.CEA+CA19-9+CA242(parallel testing):sensitivity: 100%,specificity:57.35%,positive predictive value:38.3%,negative predictive value:100%.CEA+CA19-9+CA242(series testing):sensitivity: 61.11%,specificity:94.12%,positive predictive value:73.33%,negative predictive value:90.14%.Conclusion: 1.Combined detection of CEA,CA19-9 and CA242 improve the sensitivity and other indexes than single detection for the diagnosis of colorectalcancer(no distant organ metastasis),to make up for the shortage of single detection.But sensitivity is not high,are limited valuable for the diagnose.2.Combined detection of CEA,CA19-9 and CA242 remarkable improve the sensitivity for the diagnosis of colorectal cancer with distant organ metastasis,are more valuable for the diagnose.
