Intervention Effect Of Recipe Of Enema On P38MAPK Signaling Pathway In Ulcerative Colitis With Damp Heat

Objective:This study is divided into two parts: a review and a clinical study.This part mainly describes the current progress in the understanding and treatment of Western medicine,UC and related progress on the MAPK signal pathway and traditional Chinese medicine research,the analysis of the intestinal enema prescription and study the past;clinical research mainly using the Changyu enema treatment in patients with left colonic ulcerative colitis(damp heat type),clinical efficacy to evaluate the retention enema;and to investigate the intervention effect for p38 MAPK.Method:overview part:China retrieval CNKI,China doctoral dissertations full text database(CDFD),Chinese outstanding master's degree thesis database(CMFD),PubMed and MEDLINE,and supplemented by manual search methods at home and abroad were published in the journal Proceedings.Retrieval Chinese vocabulary including ulcerative colitis,inflammatory bowel disease,intestinal enema,healing Xileisan,beauty of salad and p38 MAPK pathways,English ulcerative colitis,inflammatory key words package Bowel Disease,Enema recipe,Mesalazine P38 MAPK,signaling pathway etc..First,we read the titles and filter out the related literatures.Secondly,we extract useful information from the reading summaries and full texts in the selected literatures,and summarize the reviews.clinical study part:A total of 60 patients with UC were collected from the first affiliated anorectal Department of Anhui University of traditional Chinese medicine from July 2016 to July2017.All the selected cases met the criteria of mild to moderate ulcerative colitis,and they could be classified as damp heat and diarrhea.It was randomly divided into 30 cases of intestinal enema group(treatment group)and 30 cases of mesalazine group(control group).After using the prescribed method,two groups of patients all evaluated the therapeutic effect after 60 days.The improvement of symptom score in 2 groups before and after treatment was observed.The effect of total colonoscopy before and after treatment was compared,and the expression level of TNF-,IL-10,p38 MAPK protein and quality of life in colon tissue were compared.Results:Overview part:This includes two aspects: 1)and Western medicine for treatment of UC disease progression: Research Progress about the pathogenesis of UC in western medicine;analysis of the current situation of Western medicine in the treatment of UC;research progress of p38 MAPK signaling pathway;research on the relationship between P38 MAPK signaling pathway and UC;2)on the progress of Chinese history and understanding of the diagnosis and treatment in UC UC source: Chinese medicine TCM;UC treatment;summarize the medication rule of UC;western medicine combined treatment of TCM treatment strategies;advantages and disadvantages;correlation analysis of Changyu enema: analysis of the current research of Changyu guanchangfang composed of basic medicine.clinical study part:Reduce the treatment group and control group after treatment symptom score were two groups before treatment(P<0.05),the two groups after treatment symptom scores were no significant difference between the treatment group total colonoscopy;the total efficiency of 80%,control group total colonoscopy total efficiency of 83.3%;the 2group total effective rate was statistically significant the difference(P<0.05),the treatment group total colonoscopy efficacy than the control group;p38MAPK,TNFalpha protein in colon tissue expression levels were decreased after treatment in 2groups(P<0.05),and the treatment group after treatment significantly decreased(P<0.05);the content of IL-10,two groups of colon the quality of life scores improved(P < 0.05),the treatment group compared with the control group with or without statistically significant difference.Conclusion:At present western medicine understanding of the pathogenesis of UC is unclear,the treatment of Western medicine quick effect,but there are still some side effects of drugs and hormone dependent and hormone resistance reaction,make some patients is difficult to obtain good curative effect;Chinese medicine is broad and profound,the literature source UC long,many literatures have in-depth excavation of related content.Clinical experience play an irreplaceable role for the treatment of UC,such as our hospital experience more intestinal enema in the treatment of mild to moderate UC effect;Changyu clyster combined with Xileisan treatment of damp heat type of mild and moderate UC can significantly improve the symptoms of the disease and alleviate the damage of endoscopic mucosa,clinical curative effect is obvious;and by serological indexes,found its mechanism is probably by interfering with p38 MAPK signaling pathway to change good mucosal inflammation injury.
