Objective: In normal cells,oncogenes and tumor suppressor genes are in a stable equilibrium.When cells are affected by external carcinogens,the balance is broken,leading to oncogene activation or tumor suppressor gene inactivation,normal cell mitosis cycle.Uncontrolled regulation,uncontrolled cell proliferation,and cancerous cells.The cancer cells are in a malignant state of proliferation,and it is possible to determine whether the cells have undergone malignant transformation by detecting the proliferation of the cells.PCNA and Ki-67 are currently the main biological factors for detecting the state of cell proliferation.PCNA is a 36 KD acidic nuclear protein that is hardly expressed in quiescent cells and is expressed only in stages during the cell division cycle with cleavage ability.PCNA can assist DNA polymerase ? to synthesize DNA,participate in the repair and synthesis of damaged DNA,and play a certain role in the regulation of cell cycle.Ki-67 is another cell proliferating protein that is closely related to cell proliferation and can be expressed as a marker for recognizing normal cells and tumor cells in periodic proliferating cells.Ki-67 is expressed in stages during the cell division cycle and is involved in the regulation of cell cycle,which is closely related to the synthesis of DNA,ribosomes and nucleoli.PCNA and Ki-67 are important constituent proteins in the cell division cycle and play a positive role in the regulation and promotion of the cell cycle.Recent studies have shown that these two proteins can effectively reflect the cell division state,and have been widely used in the diagnosis of benign and malignant tumors,evaluation of tumor occurrence,development and prognosis.At present,the research on the diagnosis and prognosis of gastric cancer has been reported.However,it has been verified that there is no report on the combined detection of PCNA and Ki-67 in gastric cancer.In this study,immunohistochemical SP method was used to detect the expression of PCNA and Ki-67 in gastric cancer tissues,precancerous lesions and normal gastric tissues,and to explore the relationship between them and the development of gastric cancer and its influencing mechanism.The correlation analysis of CEA in gastric cancer;the relationship between the two and the clinical characteristics of gastric cancer cell differentiation grade,infiltration depth,lymph node metastasis,tumor location,clinical stage and so on.METHODS: Eighty-seven patients with gastric cancer who underwent radical surgery for gastrointestinal surgery at the First Affiliated Hospital of Southwest Medical University from January 2017 to December 2017 were randomly selected.All patients were diagnosed according to the standard of NCCN gastric cancer clinical practice guidelines.The specimens of postoperative cancer tissues and normal stomach tissues were well preserved,and the inpatient medical records,test and pathological data were complete.At the same time,35 cases of pathological tissue specimens of gastroscopic examination and pathological biopsy diagnosed as precancerous lesions were randomly selected from the First Affiliated Hospital of Southwest Medical University.All patients were diagnosed as chronic atrophy by gastroscopy and pathological examination.Gastritis with intestinal metaplasia or intraepithelial neoplasia.The expression of PCNA and Ki-67 in gastric cancer tissues,precancerous lesions and normal gastric tissues was detected by immunohistochemical SP method.At the same time,the gastric cancer patient test data were reviewed to understand the preoperative serum CEA expression.The experimental data were statistically calculated using SPSS19.0.The t-test was used for the measurement data,the chi-square test was used for the count data,and the correlation analysis was performed using Spearman.The specificity of the ROC curve analysis was determined by MedCalc.RESULTS: Of the gastric cancer tissues,77 were positive for PCNA,10 were negative,and the positive rate was 88.51%.Among 87 normal gastric tissues,3 were positive and 84 were negative,with a positive expression rate of 3.45%.Of the 35 cases of gastric precancerous lesions,17 were positive and 18 were negative,with a positive rate of 48.57%.There was a significant difference in the expression of PCNA between gastric cancer and normal gastric tissue(?2=22.509,126.705 P < 0.001),PCNA in precancerous lesions was significantly different from normal gastric tissue(?2=37.078 P<0.001).The expression of PCNA in gastric cancer tissues was not significantly correlated with clinical features such as gender,age,tumor location,and differentiation grade(?2=0.302,1.049,0.302,0.019 P=0.583,0.306,0.583,0.889);significantly associated with tumor lymph node metastasis,depth of invasion,and TNM stage(?2=9.621,7.206,12.978 P=0.007,0.002,<0.001).In the gastric cancer group,75 cases were positive for Ki-67,12 cases were negative,and the positive rate was 86.21%.Among 87 cases of normal gastric tissue,5 cases were positive and 82 cases were negative,and the positive expression rate was 5.75%.Among the 35 cases of gastric precancerous lesions,14 cases were positive,21 cases were negative,and the positive rate was 40%.There was significant difference in the expression of Ki-67 between gastric cancer tissues and normal gastric tissues(?2 =27.006,113.378 P<0.001);Ki-67 in precancerous lesions was significantly different from normal gastric tissue(?2=22.272 P<0.001).The expression of Ki-67 in gastric cancer group was not significantly correlated with clinical features such as gender,age,tumor location,and degree of differentiation,(?2=0.125,0.554,0.989,2.071 P=0.724,0.457,0.320,0.150);was significantly associated with tumor lymph node metastasis,depth of invasion,and TNM stage(?2=11.853,8.046,13.749 P=0.001,0.005,<0.001).According to the ROC curve analysis,PCNA is superior to Ki-67 in the diagnosis of gastric cancer.Using Spearman rank correlation analysis,the expression of PCNA and CEA in gastric cancer group was positively correlated(rs=0.682 P<0.001),and the expression of Ki-67 and CEA was positively correlated(rs=0.595 P<0.001);PCNA and Ki-67 The expression was positively correlated(rs=0.692 P<0.001).Conclusion: 1.PCNA and Ki-67 are overexpressed in gastric cancer tissues,suggesting that they may have important significance in the occurrence and development of gastric cancer.2.PCNA and Ki-67 were significantly associated with gastric cancer invasion depth,lymph node metastasis and TNM stage,suggesting that they may be involved in cancer cell invasion and metastasis.3.PCNA,Ki-67 and CEA were positively correlated,the combined detection of the three may have a certain effect on the assessment of the development and prognosis of gastric cancer.
