Objective:To investigate the curative effect of Chailing Decoction on early lower extremity swelling after artificial hip replacement;To observe the effect of Chailing Decoction on inflammatory factors after artificial hip replacement;By comparing the oral Chailing Decoction combined with enoxaparin sodium subcutaneous injection with the simple application of enoxaparin sodium,the feasibility of combined application of Chinese and Western medicine in the prevention of deep venous thrombosis(DVT)in lower extremities during perioperative period was discussed.Methods:The patients who underwent artificial hip replacement and met the inclusion criteria were randomly divided into observation group and control group with 30 cases in each group.The control group was given subcutaneous injection of enoxaparin sodium injection 3000 IU 12 hours after surgery,once daily(16 pm)for 14 days.The observation group was given Chailing Decoction on the basis of the control group.The circumference of 10cm above patella and the thickest part of the calf muscle in patients of the two group were compared at 24 hours before surgery,3 days after surgery,7 days after surgery,and 14 days after surgery.The VAS scores at rest were compared between the two groups at the first day after surgery,3 days after surgery,7 days after surgery,and 14 days after surgery.The values of C-reactive protein(CRP),interleukin-6(IL-6),D-dimer(D-D),prothrombin time(PT)and activated partial thromboplastin time(APPT)were compared between the two groups before the surgery,3 days after surgery,7 days after surgery,and 14 days after surgery.The effect of swelling treatment and the incidence of deep venous thrombosis of the lower extremities were compared on the 14th day after surgery in patients of the two group.Results:In the observation group,the circumference of 10 cm above patella on the 14th day after operation compared with on the 3rd day post operation was significantly reduced(P<0.05).In the control group,the circumference of 10 cm above patella on the 14th day after operation compared with on the 3rd day post operation was not significantly reduced(P>0.05).There was no significant difference in the circumference of the calf muscles between the two groups at each time point(P>0.05).On the 14th day after surgery,the therapeutic effect of lower limb swelling in the experimental group was significantly better than that of the control group(P<0.05).The VAS scores at rest of the observation group was significantly smaller than that of the control group on the 7th day after surgery(P<0.05).There was no significant difference in the VAS scores at rest between the two groups on the 3rd and 14th day after surgery(P>0.05).On the 7th day after surgery,the CRP value of the observation group was significantly smaller than that of the control group(P<0.05).There were no significant differences in the values of IL-6,D-D,PT and APPT between the two groups at each time point(P>0.05).No deep vein thrombosis of the lower extremities occurred in both groups after operation.Conclusion:Chailing Decoction can effectively alleviate the early swelling and pain of the affected limb artificial hip replacement and reduce the level of CRP in the postoperative blood.However,there was no difference between the combination of Chailing Decoction and enoxaparin sodium and the use of enoxaparin sodium alone to prevent deep venous thrombosis of the lower extremities.In view of its advantages,Chailing Decoction can be used for early rehabilitation after artificial hip replacement,which is worthy of promotion in clinical practice.
