| ObjectiveThis study used prospective randomized controlled clinical trials to observe changes in clinical symptoms and scores of patients before and after treatment,and to evaluate the cognitive ability of tonifying kidney and medullary combined with donepezil in patients with vascular dementia and medullary deficiency syndrome(orientation,memory,attention)Computational power,recall ability and language ability)and life ability improvement effect,objective evaluation of its efficacy,providing a clinical basis f’or the promotion of the therapy.Methods100 cases of vascular dementia(VD)patients with medullary stenosis who met the diagnostic criteria of Chinese and Western medicine were included.70 cases were systematically sampled and randomly divided into experimental group and control group(35 cases in each group).Both groups were given basic treatment(including control).Blood pressure,blood sugar control,1ipid-regulated plaque,2nti-platelot aggregation,etc.(See 2017 "Guideliries for the diagnosis and treatment of acute ischemic stroke in China").Paticrits who met the inclusion criteria were given conventional symptomatic treatment.The control group was treated with donepezil hycirochloride.The experimental group was treated with tonifying kidney and medulla on the basis of donepezil hydrochloride.Before treatment,the MMSE(simple mental state examination)scale,ADL(modified Barthel index)score and VD TCM syndrome scale(SDSVD)score were performed.After 8 weeks of treatment,MMSE,ADL and SDSVD scores were performed.To evaluate the effects of Bushen Yisui method combined with donepezil on cognitive function(orientation,memory,attentional computing power,recall ability and language ability)and life ability of dementia.Results1.MMSE score before and after treatment:Within the group,the total scores of MMSE scores in the two groups were significantly different before and after treatment(P<0.01).There was significant difference between the treatment groups and the control group(P<0.01).There were no significant differences between the two groups in the orientation and memory scores before and after treatment(P>0.05).There were significant differences in the calculation of computational power,recall ability and language ability score before and after treatment(P<0.01).Both groups can improve the cognitive ability of patients,and the combination of Bushen Yisui method and donepezil group is better than the donepezil group alone.The combination of Bushen Yisui and Donepezil can improve the cognitive ability of patients,especially in terms of attention to computational power,recall ability and language ability.2.Comparison of ADL score before and after treatment:There was significant difference between the two groups before and after treatment(P<0.01).There was significant difference between the two groups(P<0.05).Both groups can improve the patient’ 5 daily living ability.The effect of the Bushen Yishen method combined with the donepezil group on improving the daily living ability of patients was better than that of the control group.3.SDSVD score before and after treatment:Within the group,there were significant differences between the two groups before and after treatment(P<0.01).There was significant difference between the two groups(P<0.01).Both groups were able to improve TCM symptoms in patients,and the combination of Bushen Yisui method and donepezil group was superior to the control group.4.MMSE,ADL and TCM syndrome(SDSVD)efficacy before and after treatment comparison:treatment group(effective rate 84.85Y%,78.79%,84.85%)compared with the control group(effectiveness 60.00%,31.43%,20%)were significant Difference(P<0.01).In the MMSE,ADL score and TCM syndrome efficacy,the combination of Bushen Yisui method and donepezil group was superior to the donepezil group alone.ConelusionThe combination of Bushen Yisui method and donepezil in the treatment of VD myelin deficiency syndrome is effective in improving patients’ cognitive ability,activities of daily living and TCM symptoms.In terms of improving cognition,the effect on attention to computational power,recall ability,and language ability is more prominent,and it is ineffective in orientation and memory(immediate memory). |
