| ObjectiveThe acupoint selection method in this research is based on the TCM therapeutic principles of "regulating and equilibrating qi,blood,yin and yang" summarized by Prof.Zhuang Ziqi.To evaluate the efficacy of the two acupoint selection methods in treating primary dysmenorrhea,it is proposed to compare the changes of symptoms before and after treatment and the changes of corresponding clinical scale scores in different group.It provides a new idea for the acupuncture treatment of primary dysmenorrhea.Methods1.This is a randomized controlled trial.60 patients with primary dysmenorrhea were collected according to the inclusion criteria,and the patients were divided into the regulating and equilibrating group(treatment group)and the routine acupuncture group(control group)by random number method,with 30 patients in each group.There were 22 cases of qi and blood stagnation syndrome,22 cases of deficiency of qi and blood,and 16 cases of cold dampness stagnation syndrome.2.The treatment group selects shenting(DU24),qihai(RN6),ganshu(BL18),shenshu(BL23),qimen(LA14)and jingmen(GB25).In the control group,the traditional method of activating qi to resolve stagnation to relieve pain was adopted.The acupoints in the two groups were the same with syndrome matching.The treatment group and the control group were treated with acupuncture and moxibustion by acupuncture reinforcement-reduction manipulation.Treatment began 7-10 days before menstruation,and was performed once every 1-2 days for a total of 3 times,with 3 times as a course of treatment.Treatment was stopped during menstruation and follicular phase,and the curative effect was observed and evaluated after two courses of treatment.3.Scores of dysmenorrhea symptoms,visual analogue scale(VAS)and COX dysmenorrhea symptom scale(CMSS)were scored before and after treatment,followed by inter-group and intra-group comparison of the scores before and after treatment.The curative effect was evaluated by the symptom score of dysmenorrhea,and the difference in the total duration of CMSS,CMSS severity and VAS between the two groups before and after treatment was observed,so as to compare the curative effect of acupoint selection method between the two groups.Results1.Condition of shedding caseA total of 3 cases in the treatment group and the control group were removed due to loss of follow-up and shedding.There were 29 cases in the treatment group and 28 cases in the control group.2.Comparison of baseline dataThere was no statistically significant difference between the two groups in age,course of disease,distribution of TCM syndromes,VAS score before treatment,total CMSS duration and CMSS severity(P>0.05),and the two groups were comparable.3.Efficacy comparisonComparison of curative effect with course of disease and comparison of curative effect among different syndromes in the group indicated that the curative effect was not related to the syndrome type and course of disease(P>0.05).The total effective rates of the two groups were 93.1%and 85.71%respectively(P<0.05),indicating that the effect of the treatment group was better than the control group.4.Intra-group and inter-group comparisonAfter treatment,VAS score,total CMSS duration and CMSS severity of the two groups all decreased(P<0.05),indicating that both groups could improve the pain and accompanying symptoms of patients with primary dysmenorrhea.Compared with the control group,the VAS subtraction score,the CMSS total duration subtraction score and the CMSS severity subtraction score were all larger(P<0.05),suggesting that "regulating Du and equilibrating Ren acupuncture " could better improve the overall symptoms of patients with primary dysmenorrhea than the conventional acupuncture.ConclusionCompared with the conventional acupuncture prescription for the treatment of primary dysmenorrhea,"regulating Du and equilibrating Ren acupuncture"combined with acupuncture manipulation can improve the visual analogue score and COX symptom scale score of dysmenorrhea,suggesting that the former has more advantages in improving pain and accompanying symptoms,reducing duration and severity,and improving the efficacy of acupuncture in the treatment of primary dysmenorrhea.It provides new ideas for clinical treatment of acupuncture and moxibustion. |
