Objective To evaluate the application effect of continuous nursing in patients with pressure ulcers and their caregivers based on the microwebsite,to provide more effective continuous nursing service mode for patients and their caregivers to acquire knowledge related to pressure ulcer nursing,change nursing behavior,accelerate the healing time of pressure ulcer wounds,and reduce the recurrence rate of pressure ulcers.Methods The inpatients in the Affiliated Hospital of Inner Mongolia Medical University and the Second Affiliated Hospital of Inner Mongolia medical university from January to December,2018,who were diagnosed with stage III or above pressure ulcers and whose wounds had not healed when they were discharged,and their caregivers(the diagnosis was based on the 2014 edition of the quick reference guide for pressure ulcer prevention),were selected as the study subjects.Each subject was numbered and randomly divided into 5 groups,including stage 3,stage 4,deep tissue injury and non-staging,and diabetes mellitus.Subjects in each group were divided into control group and intervention group by random number table method,with 68 cases in each group.The two groups of patients or their caregivers who met the inclusion criteria were given unified instructions after admission,and they were guided to pay attention to the microwebsite platform of "pressure sore little assistant" to obtain relevant knowledge of pressure sore.Patients in the intervention group and their caregivers continued to use the platform to strengthen relevant knowledge and receive continuous care after discharge,and they could also ask questions and consult professionals in the team at any time.The patients in the control group and their caregivers no longer studied the relevant content of the platform after discharge.Monitoring on the background of the website of the microwebsite can reveal the frequency of browsing and learning of patients and their caregivers,and inform patients and their caregivers to strengthen their use through telephone follow-up.One day before discharge and three months after discharge,patients and their caregivers in the two groups were asked to fill out the Caregiver Pressure Ulcer Cognition Questionnaire,Pressure Ulcer Care BehaviorQuestionnaire and Caregiver Burden Inventory(CBI),respectively,to investigate the caregiver's pressure ulcer cognition,perceived susceptibility and severity,behavioral ability and burden,etc.Members of the continuing care group evaluated the pressure ulcers of the patients in pairs one day before discharge and three months after discharge,and filled out the Pressure Ulcer Scale for Healing(PUSH).Data were recorded and checked using EpiData3.1,and statistically processed using SPSS20.0.Results were analyzed using chi-square test,t-test,Mann-Whitney U and other statistical methods.Results(1)Mann-Whitney U test results showed that the scores of pressure ulcer related knowledge,perceived susceptibility,perceived severity and behavioral ability of caregivers in the intervention group were all higher than those in the control group three months after intervention,and the differences were statistically significant(P<0.05).(2)The t test,the Mann-Whitney U test results show that three months after intervention caregivers to take care of the burden of score evaluation,the burden of caregivers to take care of the total score,body burden score field,social areas scoring,limited development burden score difference was statistically significant(P<0.05),emotional burden field,time dependent field score difference has no statistical significance(P>0.05).(3)Wilcoxon symbolic rank sum test results showed that the scores of pressure ulcer cognition and behavioral ability of caregivers in the intervention group were significantly higher than those in the previous day after discharge,and the differences were statistically significant(P<0.05).(4)The t test,the Wilcoxon symbolic rank sum test results show that the intervention group 3 months after discharge caregiver burden in various fields scores and the day before discharge in the field of body burden score,social burden domain score difference was statistically significant(P<0.05),emotional burden score,time dependence domain score,limited development burden domain score,score difference has no statistical significance(P>0.05).(5)The Mann-Whitney U test,the Wilcoxon symbolic rank sum test results showed that the scores of pressure ulcer healing scale of the patients three months after discharge were assessed,and the scores in all fields were higher than those of the control group,and the difference was statistically significant(P<0.05).The score of pressure ulcer healing scale in the intervention group three months after discharge was significantly higher than that of the day before discharge,and the differences were statistically significant(P<0.05).Conclusion(1)The continuous nursing based on the microwebsite can effectively improve the cognitive and behavioral ability of pressure ulcer patients and their caregivers,enhance the perceived susceptibility and severity of pressure ulcer caregivers,and accelerate the wound healing time.(2)Based on the microwebsite of continuous care to reduce the journey of patients and their caregivers,reduce their economic burden,to ensure the effectiveness of the continuation,reduce the rate of lost visit.(3)The continuous nursing based on the microwebsite follows the pace of The Times,which is also the general trend of the development of media integration.This study plays a positive role in clinical practice,and at the same time provides an effective way to promote the development of continuous medical services with the medical consortium as the carrier.
