Objective To explore the pain mechanism of "Mongolian warm acupuncture therapy" in relieving sciatica and its mechanism of nerve injury in CCI model rats,so as to provide a scientific basis for Mongolian warm acupuncture therapy in treating sciatica.To investigate the regulation of temperature acupuncture in Mongolian medicine on neurotransmitters related to sciatica in rats.To verify the relationship between the expression of NE,5-HT,ACH,GABA and other neurotransmitters in the brain tissues of sciatica rats.To provide more theoretical basis and experimental data for the treatment of sciatica.To improve the theoretical basis of modern research on Mongolian warm acupuncture and enrich the modern treatment methods of traditional medicine.Methods Ninety male SD rats with body weight(200 50)g were randomly divided into blank control group,model control group,sham operation group,warm acupuncture group,acupuncture treatment group and western medicine treatment group,with 15 rats in each group.Modeling with Bennett and Xie's method.No nerve ligation was performed in the sham group.Blank control group,model control group,sham operation group: do not do any treatment.The treatment group received treatment at a fixed time every day for 15 days.In the warm acupuncture group,the Mongolian medicine silver needle was used to acupuncture "hip point" for 15 minutes,and the temperature of the warm acupuncture instrument was adjusted to 40 degrees.Acupuncture group: Mongolian medicine silver acupuncture,acupuncture hip point for 15 minutes.Western medicine group: ibuprofen was given by intragastric administration with a content of 1ml/100 g.4d,12 d,20d and 28 d after surgery,general behavior was observed,body weight was measured,field experiment,elevated cross maze experiment,thermal stimulation pain response threshold and mechanical pain threshold were tested.After 15 days of treatment,the midbrain periaquitumoral gray matter samples were taken under anesthesia with 10% chloral hydrate,and the expression regulation of neurotransmitters such as norepinephrine(NE),serotonin(5-HT),Ach(acetylcholine),GABA(gamaminobutyric acid)in the brain tissues of rats were detected by ELISA.Result: 1.General behavioral observation: the medium temperature acupuncture group was closer to the normal group than the model group,with shiny hair color,less licking of the foot,and the right hind leg of the operative side could land on the ground,and the number of feet decreased.After the operation,the injured lateral claw of the CCI group was closed in and the hind foot was slightly everted.2.Field experiment: there was a significant difference between the blank group and other groups 4 days after surgery.There were significant differences between the normal group and the acupuncture combined model group at 28 d,and no differences between the warm acupuncture group and the western medicine group.3.Elevated cross maze experiment: no statistical significance was found between each group.4.Mechanical pain threshold: there was no difference between the model group and the treatment group 12 days after surgery.There were significant differences between the model group and the normal group and the sham group.There were significant differences between the model group and the 28 d warm needle group.5.Thermal pain value: there was no difference between the model group and the treatment group 12 days after surgery.There were significant differences between the model group and the normal group and the sham group.There were significant differences between the model group and the 28 d warm needle group.6.5-HT content: there was a significant difference between the model group and the normal group and the sham group,and between the model group and the warm needle group.ACH: there were significant differences between the model group and the normal group and the sham group,and between the model group and the warm needle group.GABA: there were significant differences between the model group and the normal group and the sham group,and between the model group and the warm needle group.NE: there were significant differences between the model group and the normal group and the sham group,and there were significant differences between the model group and the warm needle group.Conclusion:1.The abnormal behaviorand analgesic effect of temperatureneedling in Mongolian medicine on rats with CCI model can be summarized according to the results of 4 postoperative behavioral experiments on rats in each group.2.The warm acupuncture of Mongolian medicine can increase the content of 5-ht and NE in the periaquitumoral gray matter of the midbrain of CCI model rats,and reduce the content of GABA and ACH to play an analgesic effect.3.The warm needle of Mongolian medicine has the characteristics and advantages of simple operation,safety,effectiveness and small side effects.
