Literature Study And Clinical Study On Diagnostic Significance Of Common Compound Pulse Signs Of Astringent Pulse

Purpose:To explore the diagnostic significance of common complex pulse of astringent pulse in ancient literature,and to analyze the distribution of etiology,pathogenesis and location of common complex pulse of astringent pulse in clinical cases,and then to preliminarily sort out the clinical diagnostic significance of common complex pulse of astringent pulse.Method:(1)Through the ancient books database and other ways to obtain the relevant literature needed to consult: Treatise on Febrile Diseases,Golden Chamber,Dan xi Xin Fa,Jing Yue Quan Shu,A guide to clinical practice with medical record,etc.nine clinical books,with the method of extracting card,systematically sort out the types of several common types of complex astringent pulse from the relevant provisions,and then sum up their respective Diagnostic significance;(2)Through the method of clinical research,taking the medical records of associate professor Shi Qiang in Qihuang outpatient department of traditional Chinese medicine from 2010 to 2018 as the research object,According to the method of clinical card extraction,the clinical diagnosis significance of complex astringent pulse was extracted strictly according to the inclusion and exclusion criteria.The above excerpt cards were analyzed by statistics.The frequency of clinical diagnosis of complex astringent pulse was statistically analyzed,and the clinicaldiagnosis significance was summarized.(3)Finally,the diagnostic significance of compound astringent pulse in literature research and the clinical diagnostic significance of clinical case statistics are mutually confirmed and supplemented.Result:Literature research results: 306 texts related to astringent pulse are extracted from the literature,of which 80 are single and 226 are compound.It is found that astringent pulse is often combined with floating pulse,sinking pulse,chord pulse,fine pulse and soft pulse from compound astringent pulse.Therefore,five kinds of complex pulse,namely,floating astringent pulse,sinking astringent pulse,stringent and astringent pulse,fine astringent pulse and soft astringent pulse,are studied.There are 154 articles on the five kinds of compound astringent pulse,including 20 articles on floating astringent pulse,59 articles on sinking astringent pulse,21 articles on string astringent pulse,38 articles on fine astringent pulse and 16 articles on soft astringent pulse.Diagnostic significance:(1)Floating astringent pulse:(1)wind-damp blocked on the surface;(2)Stagnation of solid pathogens;(3)Yin deficiency and evil prosperity;(4)Yang Qi floating.(2)sinking astringent pulse:(1)The damp phlegm stopping in internal;(2)blood stasis syndrome;(3)qi-stagnation syndrome;(4)Real Yin exhaustion;(5)stagnation of yin-cold;(6)Phlegm and blood stasis.(3)stringent astringent pulse:(1)Qi Stagnation;(2)Stagnation of blood stasis;(3)Phlegm retention;(4)Cold pathogen stagnation;(5)Blood deficiency;(6)Blood deficiency and phlegm retention;(7)hyperpyrexia of liver.(4)fine astringent pulse:(1)Deficiency of yin and blood;(2)Phlegm dampness and turbidity stopping in internal;(3)pain.(5)soft astringent pulse:(1)pathogenic dampness trapped resistance;(2)Phlegm retention;(3)Deficiency of Qi and blood.Clinical research results: There were 834 clinical cases that met the inclusion and exclusion criteria,578 cases of compound pulse of two elements and 256 cases of compound pulse of three elements.The complex pluse of three elements need to be separated.After separation,37 cases were floating astringent pulse,27 cases were sinking astringent pulse,707 cases were stringent astringent pulse,191 cases were fine astringent pulse and 88 cases were soft astringent pulse.Diagnostic significance:(1)Floating astringent pulse:(1)wind-damp blocked on the surface;(2)deficiency of positive Qi and obstruction of evil qi;(3)In the upper energizer phlegm dampness;(4)Yin deficiency and Yang floatation.(2)sinking astringent pulse:(1)Stagnation of damp turbid phlegm;(2)Damp and blood stasis are intertwined;(3)Qi deficiency and dampness pathogenic blockade;(4)Blood deficiency and stasis;(5)Blood deficiency with damp pathogen.(3)stringent astringent pulse:(1)The Phlegm and turbidity stop inside;(2)Damp and blood stasis are intertwined;(3)Deficiency of yin and blood,stagnation of pathogenic Qi(stasis)obstruction;(4)Qi stagnation;(5)Liver depression and blood deficiency;(6)Qi deficiency and pathogenic stagnation;(7)Deficiency of yin and blood.(4)fine astringent pulse:(1)Blood deficiency with damp pathogen;(2)Blood deficiency and stasis;(3)Damp and blood stasis are intertwined;(4)pathogenic dampness trapped resistance;(5)Deficiency of yin and blood;(6)Qi deficiency and pathogenic stagnation;(7)Blood deficiency and pathogenic stagnation.(5)soft astringent pulse:(1)Damp and blood stasis are intertwined;(2)pathogenic dampness trapped resistance;(3)Yin deficiency has damppathogen;(4)Qi deficiency and dampness depression;(5)Yin and blood deficiency.Conclusion:1.In literature research and clinical research,astringent pulse mostly appears in the form of compound pulse.The common complex pulse of astringent pulse is floating,sinking,stringent,fine and soft.2.The astringent pulse is a single factor pulse,only refers to the lack of fluency of pulse.The diagnostic significance of astringent pulse is a combination of the diagnostic significance of astringent pulse of single factor and its complex pulse.3.Through literature research and clinical research,it is found that:(1)Floating astringent pulse often indicates that pathogenic organisms is on the surface or on the upper-jiao;(2)Sinking astringent pulse mainly indicates dampness(phlegm)and blood stasis and other pathogenic Qi blockade or deficiency of healthy atmosphere and pathogenic stagnation;(3)Stringent astringent pulse often indicate phlegm,blood stasis and other pathogenic Qi blockade or qi stagnation;(4)Fine astringent pulse are mostly blocked by dampness and blood stasis on the basis of deficiency of yin and blood;(5)Soft astringent pulse are mostly suggestive of Damp and blood stasis are intertwined or pathogenic dampness trappe resistance.
