Objective: Studying the relationship between the risk factors of acute cerebral infarction complicated with carotid atherosclerosis and the formation and characteristics of carotid plaque in order to provide a scientific basis for risk prediction and clinical treatment of carotid atherosclerotic disease.Methods: A retrospective analysis of 247 patients with acute cerebral infarction complicated with carotid atherosclerosis diagnosed and hospitalized in the Department of Neurology,Zhongjie Hospital,Bohai New District,from November 2017 to November 2018 was applied to the research.clinical data were collected from each patient.The results of carotid artery color doppler ultrasound showed: 187 patients developed carotid plaque and 60 patients did not.According to the ultrasound characteristics of carotid plaque,the plaque group was divided into stable plaque group(87 cases)and vulnerable plaque group(100 cases).There was no significant difference in the severity of cerebral infarction between the carotid plaque group and the non-carotid plaque group,the stable plaque group and the vulnerable plaque group(P>0.05).In the patients,the possible risk factors for carotid plaque in patients with cerebral infarction were analyzed by single factor Logistic regression analysis and multivariate unconditional Logistic regression analysis to discusse the effects of independent risk factors on carotid plaque formation.Results:1.There were eight statistically significant risk factors between carotid atherosclerotic plaque group and no carotid atherosclerotic plaque group(P <0.05),including: age,sex,smoking history,SBP,uric acid,creatinine,Hcy and high density lipoprotein.There were no statistically significant differences between each other two groups(P>0.05).2.Age,smoking history,SBP,uric acid and Hcy were independent risk factors for carotid atherosclerotic plaque,ranked according to the degree of risk from high to low: smoking history(OR=3.1131,P=0.0028),SBP(OR=1.5361,P=0.0267),Hcy(OR=1.389,P=0.0161),age(OR=1.1037,P<0.001),uric acid(OR=1.007,P=0.006).3.The level of Hcy,SBP,sex and alcohol abuse history ratio in the vulnerable plaque group was higher than that in the stable plaque group(P<0.05).There was no significant difference in the other risk factors between the two groups(P>0.05).4.Sex,SBP and Hcy were independent risk factors of carotid atherosclerotic plaque,ranked according to the degree of risk from high to low: gender(OR=2.2798,P=0.0146),SBP(OR=1.3025,P =0.0131),Hcy(OR = 1.1403,P = 0.0021).Conclusions:1.Age,smoking history,SBP,uric acid and Hcy were independent risk factors for carotid plaque formation in patients with acute cerebral infarction complicated with carotid atherosclerosis.2.Sex,SBP and Hcy are independent risk factors for carotid plaque in patients with acute cerebral infarction complicated with carotid.
