Objective: With the increasing number of elderly people in China,the number of patients with acute cerebral infarction is increasing year by year.Cervical vascular atherosclerosis is an important cause of acute cerebral infarction.The more unstable the plaque is,the more likely it is to form microthrombosis,which will lead to the occurrence of acute cerebral infarction.By analyzing the relationship between the nature of carotid atherosclerosis plaque and cerebral infarction,this study explored the related risk factors,in order to achieve early diagnosis and treatment,prevent the occurrence and recurrence of cerebral infarction.Methods: 120 patients with acute anterior circulation cerebral infarction(infarction group)admitted to neurology department from June 2017 to December 2018,and 100 patients with non-cerebral infarction(control group)admitted in the same period were selected;General data,medical history and biochemical indicators of the two groups were collected,including total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),homocysteine(HCY).Carotid artery color Doppler ultrasonography was performed.Collecting relevant data and applying SPSS13.0 statistical software for statistical analysis.Results:1.There was no significant difference in age and sex between the two groups(P > 0.05),which was comparable.2.The incidence of carotid atherosclerosis plaque in cerebral infarction group and control group were 86.7% and 78.0%,respectively.There was no statistical difference between the two groups(P>0.05);the incidence of unstable plaque in infarction group and control group were 56.7% and 39.7%,respectively.The incidence of unstable plaque in cerebral infarction group was higher than that in control group,and there was statistical difference between the two groups(P < 0.05).3.The risk factors related to carotid plaque instability,such as hypertension,drinking history,smoking history,old age,TC,LDL and Hcy,were significantly higher in the unstable plaque group than those in the stable plaque group(P < 0.05),but there was no significant difference in gender,body mass index(BMI),diabetes history and TG between the two groups(P > 0.05).4.Multivariate logistic regression analysis showed that hypertension,smoking history,age,TC,LDL and Hcy were independent risk factors for carotid unstable plaque formation(P < 0.05).Conclusions: Carotid atherosclerosis unstable plaque is an important risk factor for cerebral infarction;hypertension,smoking history,age,TC,LDL,Hcy are independent risk factors affecting the stability of carotid atherosclerosis plaque;Carotid ultrasound examination can early diagnose carotid atherosclerosis,dynamically monitor the nature of plaque,and has important value in preventing and treating cerebral infarction.
