Objective: To investigate the correlation between serum cystatin C(cyst-C),homocysteine(Hcy)and carotid plaque stability in patients with large atherosclerotic cerebral infarction,and to provide a theoretical basis for the pathogenesis and prevention of cerebral infarction.Methods of study:(1)randomly selected 226 patients with type LAA cerebral infarction from the fourth people's hospital affiliated to jishou university(the first people's hospital of huaihua city).they were divided into 3 groups :86patients with stable plaque group with LAA cerebral infarction ? 40 patients with LAA cerebral infarction without plaque group and 100 with unstable plaque group with LAA cerebral infarction according to the results of carotid color doppler ultrasound..(2)personal information were collected such as age,sex,body mass index,smoking,drinking,hypertension,diabetes;serum was extracted and the concentrations of Cyst-c?Hcy?HDL?LDL?creatinine?vitamin B12,and creatinine clearance was calculated.(3)The LAA cerebral infarction unstable plaque group,LAA cerebral infarction stable plaque group,LAA cerebral infarction no plaque group,single factor ANOVA,Spearman correlation analysis and other statistical methods,to explore whether the serum Cyst-c ? Hcy level is related to carotid plaque stability.Results:(1)There was no significant difference in the basic characteristics between the three groups of LAA cerebral infarction unstable plaque group100,LAA cerebral infarction stable plaque group 86 and LAA cerebral infarction no plaque group 40.There was no significant difference in sex,creatinine clearance,BMI ? VB12 ? smoking,alcohol consumption,hypertension,type 2 diabetes,age and LDL?HDL(P >0.05).(2)The serum Cyst-c?Hcy levels in stable plaque group,unstable plaque group and non-plaque group were statistically significant between the three groups(p<0.001).The serum Cyst-c ? Hcy of unstable plaque group was significantly higher than that of stable plaque group(P<0.001),and serum Cyst-c?Hcy in the stable plaque group was significantly higher than the group without plaque(p<0.001).(3)Serum Cyst-c?Hcy levels were positively correlated with plaque instability(r=0.481,p<0.001,0.599,p<0.001)(4)The results of Logistic regression showed that serum Cyst-c?Hcy levels may be independent risk factors for plaque stability(OR=11.358;1.481).Conclusion:(1)The correlation between serum Cyst-c ? Hcy level and carotid artery unstable plaque in patients with LAA cerebral infarction.(2)Cyst-c?Hcy are a high risk factors affecting the formation of unstable carotid plaque,and their levels are positively correlated with plaque instability.
