Study On The Expression Changes Of CASC2 And MiRNA-21 In The Circulation Of Patients With STEMI Before And After Emergency PCI

Abstract/Summary:
Objective:To evaluatethe trend and significance of long non-coding RNA cancer susceptibility candidate 2(CASC2)and microRNA-21(miRNA-2)in patients with ST-elevation acute myocardial infarction(STEMI)before and after emergency percutaneous coronary intervention(PCI),to determine its potential significance in myocardial ischemia-reperfusion injury(MIRI),to explore its clinical value in the course of STEMI,We test the expressions of CASC2 and miRNA-21 in patients with STEMI before and after emergency PCI and healthy people.Methods:We selected 104 cases to be included in the study.From January 2017 to November 2017,74 patients with PCI for STEMI(STEMI group)in Weihai Central Hospital Affiliated to Qingdao University were selected as observation group,and 30 healthy people in our hospital for physical examination during the same period were selected as healthy control group.The observation group was divided into PCI preoperative group and 30 minutes after PCI group and 12 h after PCI group according to the time of emergency PCI,and the observation group were divided into MIRI group(34 cases)and non-MIRI group(40 cases)according to whether or not MIRI occurs after PCI,and their corresponding preoperative PCI patients were divided into PCI preoperative M group(34 cases)and PCI preoperative N group(40 cases).To explore the significance of CASC2 and miRNA-21 in myocardial ischemia-reperfusion in patients with STEMI,we tested the expression levels of CASC2 and miRNA-21 in the plasma of STEMI group before emergency PCI、30minutes after PCI、12h after PC Iand healthy control group,and compared the expression levels of CASC2 and miRNA-21 between STEMI group and healthy control group,compared their level changes at different time points in the STEMI group,analyzed the correlation between CASC2、miRNA-21 and cardiac troponin T(cTnT)、 high sensitive C reactive protein(hs-CRP)and their correlation.Results:1.The expression levels of CASC2 and miRNA-21 in PCI preoperative group showed an upward trend,and compared with the healthy control group,it was statistically significant(P<0.05).2.The expression levels of CASC2 and miRNA-21 in the non-MIRI group at 30 minutes after PCI were higher than those in PCI preoperative N group,but the difference was not statistically significant(P>0.05).The expression levels of CASC2 and miRNA-21 in the non-MIRI group at 12 h after PCI were higher than those in PCI preoperative N group,the difference was statistically significant(P<0.05).The expression levels of CASC2 and miRNA-21 in the MIRI group at 30 minutes after PCI were higher than those in PCI preoperative M group,but the difference was not statistically significant(P>0.05).The expression levels of CASC2 and miRNA-21 in the MIRI group at 12 h after PCI were significantly higher than those in PCI preoperative M group,the difference was statistically significant(P<0.05).There was no significant difference in the expression levels of CASC2 and miRNA-21 between PCI preoperative M group and PCI preoperative N group(P>0.05).The expression levels of CASC2 and miRNA-21 in the MIRI group at 30 minutes after PCI were higher than those in the non-MIRI group at the same time,but the difference was not statistically significant(P>0.05).The expression levels of CASC2 and miRNA-21 in the MIRI group at12 h after PCI were higher than those in the non-MIRI group at the same time,the difference was statistically significant(P<0.05).3.The expression levels of CASC2 and miRNA-21 in patients with STEMI were significantly correlated with cTnT(r=0.56,P=0.04;r=0.65,P=0.03),hs-CRP showed no correlations with CASC2 and miRNA-21(r=0.14,P=0.35;r= 0.23,P=0.49);the expression level of CASC2 in patients with STEMI was positively correlated with the expression level of miRNA-21(r=0.57,P=0.04).Conclusion:1.The expression level of CASC2 in peripheral blood of patients with STEMI is elevated,and it was involved in MIRI.2.The expression level of CASC2 in patients with STEMI was positively correlatedwith the expression level of miRNA-21.
