| Objective:The purpose of this study is to observe the clinical efficacy of adjustable lumbar cushion for the treatment of lumbar disc herniation;and determine its advantages in correcting lumbar curvature.Method:According to the inclusion and exclusion criteria of lumbar disc herniation,68 patients with lumbar disc herniation in Quanzhou Hospital of Traditional Chinese Medicine from March 20 to February 2019 were randomly divided into lumbar cushion group and control group with 34 patients in each group. The lumbar cushion group was treated with adjustable lumbar cushion for 2 months with Intermediate Frequency pulse therapy + Celecoxib capsule for 2 weeks;the control group was treated with Intermediate Frequency pulse therapy + celecoxib capsule for 2 weeks with Sleeping on a hard bed for 2 months.Observed the pain Visual Analogue Score(VAS score),the Japanese Orthopaedic Association lower back pain scores(JOA score),lumbar curvature, and the final clinical efficacy of the two groups of patients before and after 2 weeks,1 month and 2 months after treatment.Result:Four of the 68 patients were drop out of the treatment(3 in the lumbar cushion group and 1 in the control group),and a total of 64 patients were included in the data analysis(31 in the lumbar cushion group and 33 in the control group). The age, gender, duration of disease,VAS score,JOA score and lumbar curvature of the two groups before treatment were statistically tested. There was no significant difference(P>0.05),which was comparable.1 Comparison of VAS scores after treatment in both groupsComparison within the group: The VAS scores of the two groups were significantly reduced after treatment, and the difference was statistically significant(P<0.01),indicates that both treatment regimens are effective in improving pain in patients.Comparison between groups: The two groups were compared between two weeks and one month after treatment, and the difference was statistically significant(P<0.01),indicating that the lumbar cushion group had more advantages in relieving pain than the control group. There was no significant difference between the two groups at 2 months after treatment,P>0.05.Therefore there was no significant pain in the two groups.2 Comparison of JOA scores between the two groups after treatmentComparison within the group: The JOA scores of the two groups were significantly improved after treatment. The intra-group comparisons were all P≈0.000,and the differences were statistically significant. The JOA score of the lumbar cushion group increased from 14.48±3.05 to 25.81±3.66.The JOA score of the control group increased from 14.79±2.71 to 23.55±3.78.It is indicated that both groups can effectively improve lumbar function.Comparison between groups: After treatment, the JOA scores of the two groups were compared, and the differences were statistically significant(P<0.01),indicating that the lumbar cushion group was superior to the control group in improving lumbar function.3 Comparison of lumbar curvature after treatment in both groupsComparison within the group: The lumbar curvature of the lumbar cushion group was significantly improved after treatment. The curvature of the lumbar vertebra was increased from 7.75±2.19 mm to 16.23±2.64 mm.The difference was statistically significant(P<0.01),indicating that the lumbar cushion group can significantly correct the lumbar curvature. The lumbar curvature of the control group was improved 2 weeks after treatment, and the lumbar curvature was increased from 7.73±2.15 mm to 8.29±1.93 mm.The difference was statistically significant(P<0.05);1 month and 2 months after treatment The lumbar curvature of the lumbar vertebrae was significantly corrected, and the difference was not statistically significant(P>0.05).Comparison between groups: After treatment, the lumbar curvature of the two groups were compared, and the difference was statistically significant(P<0.05),indicating that the lumbar cushion group was superior to the control group in correcting the lumbar physiological curvature.4 Comparison of clinical efficacyAmong the 31 cases in the observation group,9 cases were cured,13 cases were markedly effective,6 cases were effective,3 cases were ineffective, the total effective rate was 90.32%.Among the 33 cases in the control group,5 cases were cured and 9 cases were markedly effective.12 cases were effective,7 cases were ineffective, the total effective rate was 78.79%.The difference between the two groups was statistically significant(P<0.05).The overall efficacy was better in the lumbar cushion group than in the control group. Conclusion:The adjustable lumbar cushion has achieved certain effects in the treatment of lumbar disc herniation, and can partially improve the lumbar physiological curvature, which is worthy of clinical promotion and further research. |
