In this paper,based on the view that"spleen is the main movement of water and dampness"is an important internal mechanism of fluid metabolism,the improvement effect of Jianpi Yizhi method on cerebral edema after ischemic stroke is discussed from both theoretical and experimental aspects,so as to perfect the theory of"spleen and Brain correlation",enrich the scientific connotation of"Spleen main transport"theory and provides new ideas and methods for the study of the mechanism of clinical prevention and treatment of ischemic stroke in traditional Chinese medicine.In the part of theoretical research,this paper first discusses the understanding of cerebral edema in ischemic apoplexy from the perspective of modern medicine.Secondly,from the understanding of ischemic stroke brain edema in traditional Chinese medicine,this paper puts forward the idea of reducing brain edema from the point of view of body fluid metabolism,and further puts forward that"spleen is the main transportation of water dampness"is an important internal mechanism of body fluid metabolism,spleen-stomach dominates the lifting of clearing and the lowering of turbidity,which are the general laws of lifting and lowering movement in the process of internal gasification of human body,and this physiological activity is mainly based on the spleen and stomach as the hub.Finally,the preventive and therapeutic effects of Jianpi Yizhi method on cerebral edema of ischemic apoplexy were discussed based on the metabolism of body fluid.The experimental part,the specific research is as follows:1.Objective:To investigate the mechanism of Jianpi Yizhi method on brain edema in rats with ischemic stroke and its effect on the activities of Na+K+-ATP enzyme,Ca+-ATP enzyme and free radical damage,such as MDA,SOD,CAT and GSH-Px.2.Methods:180 SPF-grade healthy male SD rats aged 13 weeks and weighing 280-300g were randomly divided into normal group,model group and traditional Chinese medicine group with 60 rats in each group after 6 days of adaptive feeding.In the model group and traditional Chinese medicine group,the middle cerebral artery occlusion(Middle cerebral artery occlusion,MCAO)model was made by thread embolization method,and the neurological deficit score was evaluated,and the rats with 23 scores were selected.2 hours after the establishment of the model,each group began to give medicine,the normal group,the model group was given normal saline,the Chinese medicine group was given Jianpi Yizhi suspension,all according to 1.44ml/100g once a day.According to the survival time,they were divided into the following five subgroups:6h,12h,1d,3d,7d,each subgroup had 12 rats.Cerebral water content was measured,cerebral infarction volume was determined by TTC staining,protein concentration was determined by BCA method,bovine serum albumin(BSA)was used as standard substance,MDA content was determined by TBA colorimetry,and protein concentration was determined by TBA colorimetry.The activity of SOD was determined by xanthine oxidase method,and the activity of CAT was determined.Under certain conditions,CAT could directly decompose its substrate H2O2,decrease its concentration,and decrease its absorbance.The determination of GSH-Px activity was used to catalyze the determination of GSH-Px activity.The reaction rate of oxidation of GSH is expressed by the decrease of GSH in unit time.3.Results:Compared with the model group,the infarct volume of the traditional Chinese medicine group decreased significantly,and the infarct size of the model group was the largest on the1st day after operation(P<0.05).In the model group,after 6 hours of cerebral ischemia,the brain water content of rats began to increase,reached the peak on the first day after operation,then gradually decreased and tended to normal(P<0.05),compared with the normal group,there was a significant difference between the two groups(P<0.05).On the 3th day and 7th day after operation,the water content of brain tissue in the traditional Chinese medicine group was lower than that in the model group(P<0.05),and reached the lowest on the 7th day after treatment with traditional Chinese medicine.6h,12h,1d,3d,after operation,compared with the normal group,the activity of Na+K+-ATP decreased significantly(P<0.05),and the activity of increased significantly at 6h,12h,7d,after operation compared with the model group(P<0.05).The activity of Ca2+-ATP in the model group was significantly lower than that in the normal group at 6h,12h,3d and 7d(P<0.05),and the activity of Ca2+-ATP in the TCM group was significantly higher than that in the model group at 6h,12h,3d and 7d(P<0.05).Compared with the model group,the content of MDA in the traditional Chinese medicine group was significantly lower than that in the model group(P<0.05),and the activity of SOD was significantly higher than that in the model group except at the 1d after operation(P<0.05).The content of CAT was significantly higher than that of the model group on the 3d and 7d after operation,and the content of GSH-Px was significantly higher than that of the model group except 6 hours after operation.4.Conclusion:(1)Spleen and stomach is the internal motive force of body liquid gasification,spleen and stomach is the armature of qi,and it is the internal motive force of body liquid operation and distribution,so"spleen main transportation"is an important internal mechanism of body liquid metabolism.(2)Jianpi Yizhi method can reduce the brain water content in MCAO rats,which is related to the increase of Na+K+-ATP activity and Ca2+-ATP activity.It is considered that Jianpi Yizhi method can improve the brain dysfunction caused by ischemic stroke by reducing the degree of brain edema.(3)Jianpi Yizhi method can promote the increase of SOD,GSH-Px,CAT expression in MCAO rats,reduce the production of MDA,inhibit lipid peroxidation,reduce the infarct area,protect the permeability and integrity of cell membrane structure,and reduce the degree of brain edema.It has obvious protective effect on the brain tissue of rats with ischemic apoplexy.
