Objective:To explore the related factors of cervical intraepithelial neoplasia CIN complicated with VAIN,in order to reduce the missed diagnosis and treatment of patients with CIN.Methods: The clinical data of patients with CIN diagnosed by pathology in gynecological ward of Liaoning Cancer Hospital Affiliated to China Medical University from January 1,2017 to December 31,2018 were collected.There were 446 cases excluding CIN or VAIN recurrence,uterine prolapse,atypical endometrial hyperplasia,malignant tumors and malignant tumors,and severe medical and surgical complications.Among them,40 were complicated with VAIN and 406 with CIN alone.Comparing the general clinical data and biochemical examination data,the difference was statistically significant according to the test level P < 0.05,and then the related risk factors of CIN complicated with VAIN were analyzed retrospectively.Results: 1.Age: The median age of the concurrent group was 53(46.25,59)years,and the median age of the single group was 44(36,50)years.The difference was statistically significant(Z =-5.17,P < 0.005).2.Menopausal status: 28 cases(70.0%)in the concurrent group and 89 cases(21.9%)in the single group had significant difference(?2 = 43.496,P < 0.005).3.The load of high-risk human papillomavirus(Hr-HPV): the median load of Hr-HPV in concurrent group was 923.4(145,2172.2)RLU/CO,and the median load of Hr-HPV in single group was 229.155(18.615,638.1275)RLU/CO.The difference was significant(Z =-2.082,P = 0.037).4.Logistic regression model suggested that menopausal status was an independent risk factor for VAIN in CIN patients,and the risk of VAIN in menopausal patients was higher than that in nonmenopausal CIN patients(OR = 8.311,95% CI: 4.062-17.005).Conclusion: The state of concurrent VAIN in patients with CIN is related to age,menopausal status and HPV load.Menopausal status is an independent risk factor for concurrent VAIN in patients with CIN.Therefore,we should pay attention to the vaginal examination and screening of perimenopausal and postmenopausal CIN patients,especially those with high HPV load.When necessary,vaginal biopsy should be performed to confirm the diagnosis and avoid missed diagnosis.
