Objective To compare the changes of serum COMP,MMP-13 and clinical efficac-y scores before and after treatment in patients with knee osteoarthritis,and to study the mechanism of sputum bone spur plaster in the treatment of knee osteoarthritis from the cytokine level.The treatment of KOA provides a theoretical basis.Methods In this study,From June 2017 to December 2018,90 patients with kne e-osteoarthritis who underwent conservative treatment in Gansu Provincial Hospital of T-raditional Chinese Medicine were selected.The KL classification was grade I-III,wh i-ch was divided into treatment groups according to the random number table method.The control group consisted of 45 patients in each group.Control group: Qizheng Xi a-otong paste external application,once two days,one piece at a time,1 course of tre a-tment for 2 weeks,continuous medication 2 courses,with publicity education,exerci s-e intervention(stripe muscle strength training,Endurance training),once two days;tre at-ment group: external application of sputum bone spur cream,once two days,one pi e-ce at a time,1 course of treatment for 2 weeks,2 courses of continuous medication,at the same time with publicity education,exercise Intervention(biceps muscle streng t-h trainning,endurance training),once two days.VAS,WOMAC and HSS scores wer eobtained before treatment,2 weeks and 4 weeks after treatment.The values of COM Pand MMP-13 in serum of patients at different time points were measured by ELI-SA,and the VAS scores of patients at different time points were compared.The diffe-renc-e between WOMAC score and HSS score and COMP and MMP-13 in patients' serum,so as to evaluate the clinical efficacy of treatment group and control group at differenttime points,and the correlation between each group,compare the cytokine val-ues and each group.The difference in ratings.Results:1.HSS and WOMAC functional scores of knee joints: Compared with before treat-ment,the HSS and WOMAC joint function scores of the treatment group and the co-ntrol group were increased 2 weeks after treatment(P<0.05);compared with 2 weeks after treatment,4 after treatment The HSS and WOMAC functional scores of the twogroups were increased in the two groups(P<0.05).There was no signif-icant differenc-e in HSS and WOMAC joint function scores between the two group safter treatment(P>0.05).2.VAS score: Compared with before treatment,the VAS pain scores of the treatm-ent group and the control group were significantly lower at 2 weeks after treatment(P<0.05).Compared with 2 weeks after treatment,the VAS pain scores of the two gr-oups were 4 weeks after treatment.There was a small decrease(P<0.05).There was no significant difference in the VAS score between the two groups afte-r treatment(P>0.05).3.Serum COMP,MMP-13 levels: Compared with before treatment,serum COMP and MMP-13 levels in the treatment group and the control group decreased 2 weeks a-fter treatment(P<0.05);compared with 2 weeks after treatment,4 after treatment The levels of serum COMP and MMP-13 in the two groups were decreased(P<0.05),and the decrease of the level III patients was the most significant.The two groups were c-ompared after treatment,and the serum COMP and MMP-13 decreased.The level tre-atment group was superior to the control group(P<0.05).4.Overall efficacy: There was no significant difference between the control group and the treatment group in relieving joint pain and improving joint function in KOA patients,but the overall effective rate of the treatment group was higher than that of t-he control group,when the serum levels of cytokine COMP and MMP-13 were decre-ased.The decrease was larger,and the control group was more likely to cause adverse reactions such as skin allergies.Conclusions longzhong bone spur ointment can effectively relieve the clinical sy-mptoms of knee osteoarthritis,its curative effect is convenient and easy to use.Its m echanism of action is to intervene in knee bone by down-regulating the expression of c-ytokines COMP and MMP-13 downstream of Wnt/?-catenin signaling pathway.The p-rogress of arthritis can significantly relieve pain,improve joint function,increase mu s-cle strength and joint stability,reduce patient suffering,and have good clinical appli ca-tion prospects.
