Objective:To analyze the relationship between ApoE gene polymorphism and coronary artery disease and blood lipids in patients with coronary heart disease.Methods:179 patients(90 patients with angina pectoris and 89 patients with myocardial infarction)who were admitted to the General Hospital of Shenyang Military Region from June 2017 to September 2018 and who underwent coronary angiography for the first time.Coronary angiography or coronary CT angiography was performed.Thirty patients who were negative were used as a control group.Patients were excluded from severe liver and kidney dysfunction,thyroid disease,malignant tumor,familial hyperlipidemia,etc.The ApoE genotype was determined by PCR-HRM method.The blood lipid level was measured by routine methods,and the patient's age and history of hypertension were recorded.Smoking history,drinking history,history of cerebrovascular disease and other clinical data and total cholesterol,triglyceride,high-density lipoprotein,low-density lipoprotein,homocysteine and other test indicators,record the number of coronary stenosis,and Gensini scoring method was used to assess the severity of coronary lesions.The relationship between ApoE genotype and ?2,?3,?4 three allele frequencies was compared between angina group(90 cases)and acute myocardial infarction group(89 cases).The relationship between gene polymorphism and myocardial infarction was compared.The gene and coronary lesion count and Gensini score were used to further analyze the correlation between ApoE genotype and the number of coronary lesions and coronary lesions.Results:1.A total of 6 genotypes(?3/?3,?3/?4,?2/?3,?2/?4,?2/?2,?4/?4)and three alleles(?2,?3,?4)were found in 179 patients with coronary heart disease.),the most common genotype is ?3/?3,followed by ?3/?4,the most common allele is ?3,followed by ?4,and the ApoE genotype of patients with coronary heart disease compared with the frequency distribution of alleles in the healthy group.There was no significant difference in the distribution of alleles.The distribution of ApoE gene in the angina group was significantly different from that in the myocardial infarction group.The ?2/?3 genotype ratio was higher in the angina group than in the myocardial infarction group.2.The TC,Hey,age and drinking rate of the myocardial infarction group were higher than those of the angina pectoris group.The TG level of the angina pectoris group was higher than that of the myocardial infarction group;the ?2/?3 genotype case group compared with the ?3/?4 genotype case group,The level of HDL-C was significantly elevated,and the above results were statistically significant.Logistic regression analysis found that age and Hey were risk factors for coronary heart disease.3.Plasma TC and LDL-C levels were increased in ?4 group compared with ?3 group.Plasma TC and LDL-C levels were decreased in ?2 group compared with ?3 group,but the difference was not statistically significant;?2,?3,?4 carrying There was no significant difference in smoking history,drinking history,history of hypertension,and history of cerebrovascular disease.4.?2 carriers had a higher proportion of coronary single-vessel disease than ? 3 carriers,while ?4 carriers had no significant difference compared with ? 3 coronary lesions;the average age of patients with multi-vessel disease was higher than that of single-vessel disease group.The alcohol consumption rate and TG level in the single-vessel disease group were higher than those in the multi-branch disease group.Logistic regression analysis showed that age was a risk factor for coronary artery multivessel disease,and high-density lipoprotein was a protective factor for coronary multi-vessel disease.5.The Gensini scores of ? 4 carriers and ? 3 carriers were higher than ? 2,but the difference was not statistically significant,suggesting that the ApoE gene polymorphism was not related to the extent of coronary lesions.Conclusion:1.The elevation of ? 3/? 4 genotype and ? 4 allele in patients with coronary heart disease suggests that it may be a risk factor for the development of chronic cardiovascular disease.2,ApoE gene polymorphism has an effect on blood lipids,? 4 compared with ? 3 and ? 3 and ? 2 compared with plasma total cholesterol,low density lipoprotein levels have an increasing trend.3.The s 4 allele was not associated with coronary artery disease and disease severity.
