Object:The relationship between macrovascular complications,serum lipid,high sensitive C-reactive protein,hs-CRP,one of the important complications of Type 2 diabetes mellitus(T2DM),was investigated in this study,which provides a theoretical basis for further searching for the occurrence and development mechanism of macrovascular disease and predicting the clinical risk of type 2 diabetes mellitus in patients with macrovascular complications and evaluating their severity.Method:60 patients with T2 DM who were hospitalized in the Department of Endocrinology from January 2019 to January 2020 in the First Affiliated Hospital of Henan University,which were divided into Type 2 diabetes without macrovascular complications(simple diabetes group)and Type 2 diabetes diabetes with macrovascular complications(diabetic complications group)according to the patients with diabetes and without diabetes related macrovascular complications.In the same period,30 healthy people in physical examination department of the First Affiliated Hospital of Henan University were selected as the control group.After fasting for 12 hours,Blood samples of all the selected subjects were collected in the morning to measure triglycerides(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),high sensitive C-reactive Protein(hs-CRP),and then analyze the changes of the above-mentioned detection indexes in each group.All data were analyzed using SPSS22.0 software.Result:1.The triglyceridesin the control group,the simple diabetes group,and the diabetic complications group were(1.07±0.37)mmol/l,(2.09±0.67)mmol/l,(2.72±1.25)mmol/l respectively;the total cholesterol in the control group,the simple diabetes group,and the diabetic complications group were(4.39±0.83)mmol/l,(5.24±1.18)mmol/l,(6.01±1.14)mmol/l respectively;LDL-C in the control group,simple diabetes group,the diabetes complications group were(2.56±0.50)mmol/l,(3.08±0.67)mmol/l,(3.71±0.56)mmol/l respectively;HDL-C in the control group,simple diabetes group,the diabetes complications group were(1.51±0.32)mmol/l,(1.27±0.38)mmol/l,(1.03±0.28)mmol/l respectively.The levels of TG,TC and LDL-C in the diabetic complications group were significantly higher than those in the simple diabetes group and control group by statistical analysis,and the levels of HDL-C were significantly lower than those in the simple diabetes group and control group,the levels of TG?TC?LDL-C in the simple diabetes group were significantly higer than those in the control group,the levels of HDL-C were significantly higher than those in control group,the differences were statistically significant(P<0.05),which showed that there was lipid metabolic disorder in patients with Type 2 diabetes,and suggested that dyslipidemia may be closely related to theoccurrence and development of Type 2 diabetes and its vascular complications.To a certain extent,it has certain predictive value for the risk and severity of macrovascular complications in patients with Type 2 diabetes.2.The levels of hs-CRP in the control group,simple diabetes group and diabetic complication group were(1.43 ± 0.66)mmol / L,(4.58 ± 1.068)mmol / L,(6.80 ± 2.93)mmol / L respectively.The level of hs-CRP in the diabetic complication group was significantly higher than that in the simple diabetes group and the control group by statistical analysis,and the levels of hs-CRP in the simple diabetes group were significantly higher than that in the control group(P < 0.05).3.The correlation analysis result between the hs-CRP and serum lipid showed that the correlation index r of hs-CRP and TG,TC,HDL-C,and LDL-C were 0.403,0.616,-0.449,0.551 respectively,which indicated that there were a correlation between hs-CRP and the indexes of serum lipid,of which there was a significant positive correlation between hs-CRP and TG,TC,LDL-C(r>0,P<0.05),and there was a significant negative correlation between hs-CRP and HDL-C(r<0,P<0.05).4.The ROC curve analysis results of various indicators for in diagnosing macrovascular complications were that The areas under the curve of TG,TC,HDL-C,LDL-C,and hs-CRP were 0.672,0.683,0.727,0.772,and 0.748 respectively,and the corresponding P values were less than 0.05,which indicated that each index had diagnostic value for diabetic patients with macrovascular complications.The threshold value of TG was 2.26,the corresponding sensitivity was 66.7%,the specificity was 73.3%;the threshold value of TC was 5.06,the corresponding sensitivity was 80.0%,the specificity was 56.7%;the threshold value of HDL-C was 1.34,the corresponding sensitivity was 86.7%,the specificity was 56.7%;the threshold value of LDL-C was 3.175,the corresponding sensitivity was 90.0% The sensitivity and specificity of hs CRP were 66.7% and 76.7% respectively.It is suggested that LDL-C had the highest sensitivity and hs-CRP had the highest specificity in the diagnosis of Type 2 diabetes with macrovascular complications.Conclusion:1.There was a significant positive correlation between hs-CRP and TG,TC,LDL-C(r>0,P<0.05),and a significant negative correlation between hs-CRP and HDL-C(r<0,P<0.05).2.The each index of hs-CRP and serum lipid had diagnostic value for diabetic patients with macrovascular complications.LDL-C had the highest sensitivity and hs CRP had the highest specificity.It can be seen that hs-CRP and serum Lipids may be able to predict the risk of macrovascular complications in patients with Type 2 diabetes to a certain extent,and provide data references for the prevention and treatment of Type 2 diabetes and its various complications.
