| Objective:The incidence of malnutrition in maintenance hemodialysis patients and the influencing factors of malnutrition were analyzed.Methods:88 non-inpatients who underwent maintenance hemodialysis at the First Affiliated Hospital of Kunming Medical University and had a dialysis age greater than 3 months were selected to evaluate the adequacy of dialysis.The q-uantitative global assessment(Q-SGA)method of dialysis patients was used to assess the nutritional status of patients,Perform anthropometric measurements on patients,and simultaneously detect biochemical indicators such as hemoglobin,albumin,and total cholesterol;use spss17.0 statistical software package for statistical analysis.The difference of P<0.05 was statistically significant.Results:1.Q-SGA evaluated the nutritional status.The results showed that among 88 maintenance hemodialysis patients,61 cases(69.32%)were malnourished,58 cases(65.91%)were mild to moderate malnutrition,and 3 cases(3.41%)were severe malnutrition.).2.Comparison between the well-nourished group and the malnourished group found that the differences in albumin,blood creatinine,triglyceride,high-density lipoprotein cholesterol,phosphorus,hemoglobin,urea clearance index(KT/V),and dialysis age were statistically significant(P<0.05),and urea nitrogen,uric acid,retinol binding protein,blood sugar,total cholesterol,free cholesterol,low density lipoprotein,calcium,transferrin,daily protein intake,daily energy intake,There was no significant difference in age and gender between the two groups(P>0.05).3.The comparison of anthropometric indexes between the well-nourished group and the malnourished group showed that the difference in grip strength,upper arm muscle circumference,upper arm circumference and body mass index was statistically significant(P<0.05),while the thickness of the triceps skin fold between the two groups was There was no significant difference in body weight(P>0.05).4.Multi-factor analysis of malnutrition found that the age is high(P<0.01,OR=1.07,95%CI:1.04-1.10),low level albumin(P<0.01,OR=1.15,95%CI:1.07-1.25),hypophosphatemia(P=0.03,OR=3.55,95%CI:1.56-8.06),dialysis age(P=0.03,OR=1.14,95%CI:1.05-1.25)risk of malnutrition Higher,KT/V ≥ 1.2(P=0.049,OR=0.41,95%CI:0.17-0.99),high body mass index level(P=0.024,OR=0.18,95%CI:0.04-0.8)malnutrition The risk is lower.Conclusions:1.The incidence of malnutrition in patients with MHD is high;2.There are many factors that affect malnutrition in MHD patients;3.Old age,low levels of albumin,low phosphorus,and long dialysis age are risk factors for malnutrition.Kt/V≥1.2 and high level body mass index are protective factors for malnutrition.They can be used to monitor the nutritional status of MHD patients. |
