Objective Patients with moderate-risk prostate cancer with different major grades(Gleason score 3 + 4 and Gleason score 4 + 3)have significantly different prognosis.Based on this,patients are divided into two types with good prognosis and poor prognosis.To investigate the correlation between contrast-enhanced ultrasound parameters and transforming growth factor beta 1(TGF-?1)in patients with prostate cancer Gleason 3 + 4 and Gleason 4 + 3.Methods 87 patients with moderate-risk prostate cancer(Gleason = 7)underwent transrectal contrast-enhanced ultrasound and prostate biopsy.According to the histopathological results,they were divided into Gleason3+4 group(n = 42)and Gleason4+3 group(n = 45).Compare the two groups of contrast-enhanced ultrasound modes,contrast-enhanced ultrasound parameters and the expression of TGF-?1 in prostate tissues,analyze the correlation between the contrast-enhanced ultrasound parameters and TGF-?1,and the effectiveness of distinguishing Gleason 3 + 4 from Gleason 4 + 3 prostate cancer.Results 68.9%(31/45)of prostate cancer Gleason 4 + 3 group and 47.6%(20/42)of Gleason 3 + 4 group showed a increased contrast ultrasound mode(P <0.05);the Peak Intensity(9.60±2.81)and Area Under the Curve(606.93 ±284.55)of contrast-enhanced ultrasound in Gleason 4+3 group were higher than those in Gleason 3+4 group(7.24 ±2.19,412.53 ±164.35)(P < 0.05).The high expression rate of TGF-?1 in prostate cancer of Gleason4+3 group(55.6%)was higher than that in Gleason3+4 group(31.0%)(P < 0.05).TGF-?1 was positively correlated with Peak Intensity(r = 0.537,P < 0.001),Mean Transit Time(r = 0.281,P < 0.008),area under the curve(r = 0.607,P < 0.001),and Half Time of descending(r = 0.380,P < 0.001),but not with Rise Time of the ultrasound contrast parameters(r = 0.124,P < 0.254),Wash In Slope(r = 0.126,P < 0.243),and Time To Peak(r = 0.076,P < 0.485).When the best cut off values of lesion Peak Intensity,Area Under the Curve were > 9.19,> 582.84 and TGF-?1 was highly expressed,the AUC to distinguish prostate cancer histological scores Gleason 3+4 and Gleason4+3 were 0.753(95CI:0.649~0.839,P<0.001),0.730(95CI:0.624~0.819,P<0.001),0.612(95CI:0.501~0.75,P<0.001)P < 0.001),and the sensitivity and specificity were 62.2% and85.71%,53.33% and 92.86%,53.33% and 69.05%?Conclusion Contrast-enhanced ultrasound parameters can reflect the vascular perfusion of prostate cancer.TGF-?1 is mostly overexpressed in Gleason 4 + 3 prostate cancer.Contrast-enhanced ultrasound parameters combined with TGF-?1 have certain value in assessing the prognosis of patients with moderate-risk prostate cancer.
